To upload proof of your immunity, upload your records to the Patient Portal on The Ashe Center website. IMMUNIZATION REQUIREMENT TIP: In addition to the required vaccines (DUE OCTOBER 1), you are required to answer the screening questionnaire to determine if you need to complete testing for tuberculosis. To find the questionnaire, visit www.studenthealth.ucla.edu and click on ‘Patient Portal’.
