The forms are done through our booking app and if for any reason we are asked to show these forms they will be accessed through our private booking program. They aren’t for our personal use & are required in California for the time being. Please feel free to fill it out now. If you choose not to sign the form please feel free to reschedule for a later date once the Covid requirements begin to be lifted. Hello everyone just to clarify the COVID-19 tracing waiver is a questionnaire asking whether or not you or anyone you’ve been around has had Covid or hand symptoms of Covid along with the release of liability and stating that you are aware that you need to notify if you have or have been around anybody that has had it.

Here’s some of the new rules being put into place here @Shear Madness Hair Design. Please be patient with us as we are entering a new way of life. Contact your stylist to see what their schedule is to book and appointment. We are working with each other to be able to follow the orders set for us. 1) If you or your stylist are experiencing cold or flu like symptoms within 14 days of you appointment your appointment will need to be rescheduled. Temperatures of all stylists & p...ositions will be checked upon entrance. 2) You will need to fill out a Covid tracing waiver and have your temp taken before entrance. 3) Face coverings are to be worn by stylist and patron during entire service along with fresh capes. 4) You and your stylist will wash your hands before service to ensure proper sanitation. 5) Please notify your stylist when you arrive and they will let you know when they’re ready for you to enter the salon. Consider this upon arriving to early. 6) Do not arrive late or you will be rescheduled. We have to use every second of our time in between to properly disinfect before each client which is already extending our day. No exceptions. 7) No double booking or group appointments. If you’re used to bringing someone with you or your children to be serviced between processes we can no longer do this. Everyone else will have to wait outside or in the car. You can book for separate times. 1 client per 1 stylist at a time. No purses, bags, or backpacks. Please only bring keys, cell phone, form of payment, & a book for longer processes. We will no longer have magazines for you to look at. 9) We are no longer allowed to offer snacks or beverages at this time so feel free to bring your own. Please be patient with us as we are following the CDC guidelines along with California Board of Barbering and Cosmetology. Please contact your stylist to see what forms of payment they’re accepting and when they will be returning. Some will still be social distancing or need to find child care.
