We’re looking for a Registered Nurse to join our team. ***$5,000 Sign On Bonus*** You will report directly to the Administrator, Executive Director, or Patient Care Manager Senior and will maintain primary control and professional management of each patient. You will act as primary liaison between physician, patient/family, and hospice team.... You’ll provide direct patient care services to hospice patients and supportive care to the patient’s family with a reasonable caseload You’ll manage an interdisciplinary group consisting of Licensed Practical/Vocational Nurses, Certified Nursing Assistants and support staff to enhance the quality of hospice care You’ll evaluate patient/family needs and assist in the preparation and execution of the plan of care for each patient You’ll maintain all assigned patient clinical records You’ll participate in company quality assessment and performance improvement programs About You Compassionate, kind and empathetic to your patients and their families A current RN license (in the state of requested employment or ability to obtain) 1+ years of nursing experience in hospice, home health, or oncology preferred This role requires full COVID-19 vaccination. We're looking for a Patient Care Manager to join our team. ***$10,000 Sign On Bonus*** ... You will report directly to the Executive Director, Administrator, or Senior Patient Care Manager. You will be responsible for the day-to-day operations of your assigned hospice site, administering the clinical aspects of the hospice program, and for ensuring the provision or quality of care to patients. Managing the site in the absence of the Executive Director, Administrator, or Senior/Executive Patient Care Manager. Managing and overseeing clinical and administrative, and recordkeeping functions Responding to phone referral/inquiries in a timely, professional, and compassionate manner Participate in on-call nursing schedule and providing direct skilled bedside nursing care in patients homes About You Degree in Nursing. Registered Nurse with ASN or BSN from an accredited school of nursing, one year experience as an RN (Licensed in the state of requested employment) Three years' experience in direct patient care. One year of management experience. Two or more years' experience in hospice or home health required. Must be able to effectively operate nursing assessment tools and office equipment to include, computer and or tablet Sensitivity to feeling of the terminally ill and their families before and after a patient's death Knowledge of terminally ill patients and their families along with an understanding of the hospice concept This role requires full COVID-19 vaccination. The coronavirus continues to take its toll on communities across the U.S., creating financial hardships that make putting food on the table a struggle for many. In the past week, we’ve raised an additional $4,600, bringing our total so far to $13,112that’s more than 130,000 meals!
As we move into our final week of the food drive, we have exciting news to report about the donations we’ve already raised. Almost $8500 in 3 weeks. That’s about 102,000 pounds of food for communities across the country!
