Santoro, MD Assistant Professor of Neurology and Director of Neuroimmunology and Demyelinating Disorders Children’s Hospital Los Angeles and Keck School of Medicine at USC... Study: The Impact of the Developing Endocrine System on Pediatric Onset Multiple Sclerosis Disease Course: Multiple sclerosis (MS) has onset less than 18 years of age in approximately 5% of all patients. Children with MS have been previously identified as having more relapses than adults and risk factors include obesity and earlier onset of puberty. Further, female predominance is not observed until after 12 years of age, indicating that sex and stress hormones may have a role in these early onset cases. Our study looks to identify the impact of sex hormones (such as estrogen and testosterone) and stress hormones (such as cortisol and adrenocorticotropin) on disease severity and symptomatic issues such as fatigue, sleep dysregulation, and depression. Should these hormonal biomarkers indicate greater disease severity, they may be utilized for prognostic purposes and may provide a starting point for evaluating hormonal impact on the use of disease modifying therapy. The team at CHLA has established one of the fastest growing pediatric-specific MS centers in the country and are dedicated to improving the lives all of children with MS. Medical Monday is back! Q: Is it safe for me to get the COVID-19 vaccine? A: The availability of Covid-19 vaccines is very exciting. The 3 vaccines approved by the FDA are mRNA vaccines (made by Pfizer and Moderna) and adenovirus based vaccine (made by Johnson and Johnson). None of these vaccines are live vaccines. They all appear highly effective at preventing Covid-19 infection (and admission to the hospital due to Covid) with an overall excellent safety profile. Common sid...e effects around the time of vaccination include fatigue in up to 10% of individuals and transient fever in up to 2% of individuals. A few things to note is that patients with MS were not enrolled in the vaccine trials and as such we do not have specific data on the safety of these vaccines in patients with MS. However, based on the mechanism of action of these vaccines and the many patients with MS vaccinated in the past few months, we do not anticipate Covid vaccine increase the risk of relapse or MS progression. Of note, patients with some MS disability at baseline may experience transient worsening of symptoms if they develop fever due to the vaccine (which is call pseudo-exacerbation). Symptoms return to baseline within hours and improve with medications which decrease fever such as Tylenol or Advil. Some medications used to decrease MS progression such as rituximab, Ocrevus, Kesympta, Gilenya, Mayzent, ozanimod, Mavenclad and alemtuzumab may dampen the effectiveness of vaccines in general, so we expect this would also occur with Covid-19 vaccines. For all these drugs, we recommend to wait 4 weeks or more after completing the vaccination schedule to start treatment. If already on rituximab, Kesympta or Ocrevus, we recommend to wait 4 weeks or more after vaccination to get the next dose. See more

Flashback Friday! The first-ever RACE to Erase MS was held in Aspen, Colorado just one year after chairman @nancypeaceandlove was diagnosed with the illness on a ski vacation. The two-day event which took place February 14 & 15, 1993, raised $1.3 million for the MS research efforts of Dr. George Ellison of UCLA, The Rocky Mountain MS Center and The Aspen Valley Hospital.... The pro-celebrity ski race held in conjunction with the Chrysler Pro Ski Tour included stars MC Hammer who performed at the Country Western Party Friday evening, Michael Bolton and Kenny G performed Saturday night and celebrities in attendance included Dennis Miller, George Hamilton and Jimmy Connors. And we can’t forget the snow volleyball tournament that landed more snowballs than spike balls! #racetoerasems

The renowned band will rock the bowl with a full set and help raise funds and awareness for multiple sclerosis, this life-altering autoimmune disease. The landmark event will also boast a Spring/Summer 2021 runway show from Hollywood-favorite fashion brand alice + olivia by Stacey Bendet. Additional special guests and event details will be announced in the coming months!

Here’s your weekly dose of Medical Monday! Q:I live with MS. How do I chose the right activity and exercise? A: Exercise is known to have many benefits in MS. For example, there is data to support that exercise is associated with greater quality of life (QOL), cognition, and help fatigue. Data to support its benefit on fatigue and QOL comes multiple systematic review and meta-analysis that find improvement on fatigue measures and quality of life. Yoga, Pilates, and other fo...rms such as low intensity exercise have all been found to be beneficial. Exercise also likely plays a positive role in your immune system. Data support that exercise reduces inflammatory stress which could be an important mechanism in MS. Other indirect benefits are that exercise can prevent weight gain and decrease cardiovascular comorbidities which are known to be bad in MS (because they can increase the risk of progression). Such comorbidities include hypertension and diabetes. These two comorbidities are currently known to increase the risk of developing greater complications if infected with COVID-19.
