As an Endodontist, many of our patients are considered emergency due to pain and infection. However, many of these patients may benefit from appropriate prescription medications and minimize unnecessary dental visits. These are always challenging ( and yet so rewarding when you locate the canal )As always , CBCT is invaluable here. 04 taper. WVC , orifice sealed with resin and internal bleaching placed. Probably could’ve made a smaller opening in the enamel ( guess I’m still a chicken ) but I’ll take it if it helps preventing iatrogenic mishaps. Happy Friday ! Another great example of how CBCT simplifies life. Pre op radiograph does not show anatomy of #2 adequately. We took a pre op CBCT and we’re able to identify all four roots and treat them successfully. Lower premolars are my second least favorite teeth ( after lower anteriors ). This is why. #20 SIP and SAP One visit CT reveals complex anatomy - particularly in the buccal root... C/S two main canals with rotary and handfiled with stepback the third branch. Endoactivator used through out and also used to apply sealer in the buccal. 2 visits. 4 hours. Let’s save teeth. Crown and decayed removed. BU with GI. Post and GP removed and CAOH placed for 4 weeks. 2nd session : More disinfection. Obturation using hiflow BC sealer (any sealer would be okay here IMO but Ive switched to BC sealer for all my retreatment cases and that’s just my personal choice of material ). I don’t use the single cone method. For larger canals and unusual anatomy I’ll laterally condense what I can and then use warm vertical methods to downpack and backfill. Diagnosis is always the toughest aspect of Endodontics and days that we can’t figure out why someone is having tooth pain is frustrating for an endodontist. Luckily CT scans help aid in diagnosis ( not a replacement for clinical testing) This case is a great example. The patient presented with CC of vague intermittent pain in the right side. All my Endodontic tests were normal initially. I could not find anything abnormal radiographically and clinically. A CBCT was taken.... On the CBCT it’s obvious that #3 has something wrong. But we still need to verify clinically and also reproduce symptoms. After the CBCT I decided to cold test #3 from the palatal aspect and was able to reproduce the patients chief complaints. We’re only as good as the sealer we displace :) C shape canals are probably the biggest challenge to disinfect properly. According to Weine 8% of mand second molar are C shaped. Significant time disinfecting with ultrasonics and obturation using high flow BC sealer. Handfile, pathfile, rotary file, help file, pray file - you name it , I used it One visit , obturated with AH+ And orifice sealed with GI. It’s when we see these intricate like systems obturated that we appreciate how complex root anatomy is.
