Is Our Fear-Based Approach Towards COVID-19 Naïve, Wrong, and Misplaced? Waiting for a vaccine to protect the elderly and high-risk persons is not a rational strategy! If we are to open schools and businesses safely, we need a strategy that protects the high-risk population now!

Can We Open Schools in the U.S. Safely? THE BAD NEWS: We are 20 times worse than our EU neighbors in positive cases AND death rates. Our rising deaths will very likely get worse over the next 2 weeks, so we must begin reversing the trend immediately.... Can we please agree on mandatory face masks in areas with rising infection and hospitalization rates? If we do this, there is a chance that by end of August, the rising trends will be reversed and we can reopen most schools across the country in a safe and metered way. THE GOOD NEWS: 99.2% of COVID 19 cases in the USA are labelled as not serious cases. These data are similar to worldwide data (98.8% of cases not serious). (Sourced from Worldometer database: https://www.worldometers.info/coronavirus/country/us/.) USA (1.9 million cases; 16,000 serious cases (0.8% or 1:1,250 cases). Worldwide (4.9 million cases; 59,150 serious cases, (1.2% or 1:833 cases). ADULTS ARE THE ONES AT RISK: As of July 8th, CDC noted that 92% of all COVID-19 deaths are above age 55, and 80% above age 65%. The majority of serious illness occurs in patients with multiple pre-existing conditions (i.e., obesity, diabetes, hypertension, heart disease). NOT YOUNG CHILDREN, TEENAGERS OR COLLEGE STUDENTS Only 14 deaths in children under age 15 (1:8,333 COVID 19 deaths). Only 149 deaths in ages 15-24 (1:130 COVID 19 deaths). EDUCATE THE AT RISK ADULT POPULATION At risk adults need to be educated in reducing their risk! When kids go back to school, some at risk adults and teachers will undoubtedly be exposed to COVID-19. WHY NOT IMPLEMENT A NATIONAL EDUCATION CAMPAIGN ON HOW TO REDUCE RISK OF SERIOUS COVID19 ILLNESS IN ADULTS? The medical community has a lot of preventive medicine principles to share!!! This is to save our nation’s economy and schools, WAITING FOR A VACCINE IS NOT A RATIONALE STRATEGY! Public Health is not limited to social distancing and masks. These are our first line of defense. I don't care if you’re Republican, Democrat, Libertarian or vote for unicorns. If you're not asking these questions you should be. These are the important quest...ions folks need to be asking themselves! 1. Why can you go to Walmart but not Kohl's? 2. Why the Dollar store and not a mom-and-pop shop? 3. Why can't you have an elective surgery, but you can have an abortion which is elective? 4. Why should you stay inside but yet heat and sunlight kill the virus? 5. Why do the homeless consistently demonstrate the lowest infection rates? 16. Why are they telling us to mask up after 2 months of lockdown? 17. Why is the CDC saying kids need to be masked when they return to school or attend church, when they know cloth masks restrict oxygen? 18. What is this oppression and loss of liberty doing to the mental health of our kids...to us? 19. Why have most other death rates dropped since the virus? 20. Why did world leaders meet in China in October 2019? 21. Why are the common people being controlled by the government and no one is controlling the government? 22. Why are hospitals paid more for Covid 19 deaths? 23. Why are some doctors speaking out and then getting silenced? 24. What does a computer geek have to do with a pandemic and why does he want 7 billion corona virus vaccines? 25. Why did the CDC have a job posting for pandemic relief workers in November 2019? 26. We Should Prepare for the Future By Focusing on Us and Not the Virus According to the USA data today, in all States, the median (mid-range) number of deaths per one-million population is 42 deaths per million (DPM). California is slightly lower at 39 DPM, while NY State has 1,063 or 28 times more than California. So, regional differences require regional solutions. An example of states restarting their economies include Texas with 20 DPM, South Carolina with 30 DPM, Florida w...ith 47 DPM they are all fairly low. However, Georgia is the outlier at 85 deaths per million! We hope that Govenor Kemp has made the right decision! Random antibody testing in New York suggested that 14-21% of people were COVID-19 positive. This means that in the highest testing State of New York, 10-times more people are infected than now known, and the fatality rate is roughly 10 times less, about 0.5%. THE IMPORTANCE OF PRE-EXISTING CONDITIONS: For the few people that have experienced severe disease and subsequently died, the importance of pre-existing conditions is illustrated below. It is rare to die of COVID-19 without pre-existing conditions. According to the JAMA article "Presenting Characteristics, Comorbidities, and Outcomes Among 5,700 Patients Hospitalized With COVID-19 in the New York City Area," 88% of 5,700 patients hospitalized patients in NYC had greater than one comorbid condition, and half of them had four or more (the highest numbers are with hypertension, diabetes, and obesity); 84% NEVER smoked; only 9% had asthma; and 5% had COPD. This virus does not target patients with pre-existing lung problems, but rather those with pre-existing tendencies towards inflammation. The way the virus enters the body via the ACE-2 receptor explains all of this and gives us our main lesson: THE BEST WAY TO PREPARE FOR THE FUTURE, ANY FUTURE, IS TO FOCUS ON US, THE HOSTS, NOT THE VIRUS!!! In the USA - the Sky is NOT Falling! When we compare USA mortality with the top European countries, look at what we see in Chart 1! When you look at USA State data, look at what we see in Chart 2!... We have REGIONAL challenges in the USA, and even in NY, new hospitalizations but our best measures of future deaths are declining! Look at Chart 3! Within the USA, the top 10 states with 150 million population have 81% of the total USA deaths (1 per 12,800 persons). Remaining 40 states with 180 million population have 19% of all deaths (1 per 65,800 persons). With these relatively low-risk numbers in much of the USA, we NEED TO TEST A RANDOM SAMPLING OF THE POPULATION TO UNDERSTAND THE INFECTIVITY RATE! WE NEED TREATMENT OPTIONS! There are 306 clinical trials in the US Clinical Trials Database examining many options, including: Hydroxychloroquine and Zithromax for treatment and prevention of both patients and healthcare workers, the use of Vit C, Vitamin D, Hyperimmune Plasma, Immune modulators that block cytokines, the use of anti-oxidants, anti-virals, and interferon. Clinical trials using umbilical stem cells and exosomes are also underway.... The COVID-19 pandemic will bring these obvious ideas to strengthen and modulate our immune systems to help all of us in the future. Acute lung injury from a virus or bacteria or shock will be treatable in new ways in the future because of results from these COVID-19 clinical trials because the mechanism of injury is largely the same hyper-inflammation. I hope VERY SOON to see the number of clinical trials in the USA match the numbers now from China and Europe! We need two things in the USA to get back on track: 1) treatment options to keep persons out of critical care; and 2) rapid and comprehensive testing. Trial on stem cells in COVID-19 enters Phase 2 A clinical trial using stem cells to treat COVID-19 patients enters phase 2 and is underway in China. Researchers from numerous hospitals around China have teamed up to conduct a multicenter trial to assess the effectiveness of stem cell therapy in treating patients with severe COVID-19. This trial has entered phase 2 after their preliminary data demonstrated that stem cell therapy was safe in COVID-19 patients.... Of the 90 patients that are to be recruited for this trial, 60 will receive intravenous stem cells, and 30 will receive a placebo. All patients will receive conventional care during the trial. During the 90-day follow-up period, patients will be assessed multiple times, primarily for changes in the severity of pneumonia and lung fibrosis, as well as oxygenation levels, length of hospitalization, and side effects. Preliminary results have not been published, and this trial is estimated to be completed later this year. While we wait for the results of this trial and others, stem cells are becoming more promising for COVID-19 as two clinical trials have already demonstrated positive effects.
