A lot of time, effort, finance and other priceless sacrifices go into even the smallest innovation and inventions that contribute to society’s progress. If inventors should fear that other parties can swoop in after all the work has been done to claim credit, and all the commercial benefit, we would be punishing rather than rewarding true inventors. Innovations would be discouraged and it is the society and in this case FUE providers and patients that loose. Finally our hair ...societies, workshops etc should be professional mediums in which the free sharing of new ideas is encouraged and not suppressed. Innovators and inventors should feel free to share their protected ideas with colleagues without fear. This of course is not correct as only about 20% do so. The FUE Shave Test - FST is a tool used to separate patients that would go on to develop severe baldness from the majority who would not. It is a more reliable measure of the SDA and can be called Dynamic SDA comp...ared to the former version which is Arbitrary and can be called Static SDA. PUNCH SIZES AND WOUND IMPLICATIONS OF HYBRID PUNCHES FUE Hair Transplant: The new generation of punches which are flared so the cutting tip points away from the follicle are becoming popular. There are various types of these #hybridpunches as this video indicates. https://www.youtube.com/watch?v=cypAiW1RzNI... While the hybrid punches offer immense advantages over traditional straight edged punches there are differences between on many levels, one of which is the effective cutting tip diameter and the wound sizes. Here is how to analyze the differences A. The earlier generation of these punches (left in photo) have a trumpet shape where the punch cutting tip is flared by expanding the diameter of the tip compared to the rest of the diameter. B. The Next Generation of punches (right on photo) achieved the flared (hybrid) effect by having the concavity carved out of the wall of the punch. The result is the flare and (away facing cuts) is achieved without increasing the punch diameter. Comparing the two, at full capacity: i. The first generation (A) would need to cut a bigger wound to retrieve the same graft that (B) can retrieve. Another ii. The first generation #hybridFUEpunches (A) would bite more than they can chew in certain skin types and this would mean more tissue is squeezing its way up the lumen of the punch resulting in increased friction, impaction and transection. It is for this reason that these punches can only perform best in oscillation. In continuous revolutions, the transection rate of these punches would increase. Biting more than they can chew is also the reason they would have a higher incidence of buried grafts unless they are very sharp.

FUE PUNCH SIZES: OVERDUE FOR STANDARDIZATION The other day, a visiting Dr at my clinic remarked upon studying my punch tip and wounds that what I am calling a 1.0mm punch in my clinic is likely equivalent to a 0.90mm or less at their clinic. This highlighted in my mind the need to establish standards for FUE punch size measurements and reporting of them. The subject of punch sizes is currently murky:... 1. There is no clarity on what part of the punch must be measured when reporting punch sizes. Should it be the outer diameter, the inner diameter? Is it the cutting tips or the cutting ends? Are we reporting the actual punch tips or the drill bits used in creating them? Without an established standard, all discussions comparing punches would be incoherent. 2. There is a tendency to minimize punch sizes by manufacturers and clinics as the perception is that a smaller punch is better for PR 3. Depending on the type of tool used to measure, you can get different measurements on the same punch. 4. The wound profile and graft profile is affected by the punch design, so two punches of the same size can leave different donor wound profile. Furthermore, newer punches may have unusual shapes that call for clarification regarding method of action and how best to determine their sizes. The following video is an attempt to open up the discussion on this subject. Hopefully, it would trigger a change in the industry. A standard is needed so we can be consistently coherent in our clinical practice, research and publication activities on #FUEPunchesizes

At the end of Day 2 after a full day of hands on cadaver didactic, certificates were issued to the attendees some of whom posed for a photograph. The night saw some of the attendees and faculty going to see Jay Leno's stand up performance at the Hermosa Beach Comedy and magic Club
