What is done during an #AFib ablation? The #arrhythmia itself is not the primary target - the triggers for the arrhythmia are. If AFib is a fire and the triggers are the sparks, ablation involves insulating the sparks from causing the fire. In most cases these sparks originate from the pulmonary veins in the left atrium. The most common reason for AFib recurrence after ablation is regrowth of tissue. However, as technologies improve this is happening less often. Extr...a-pulmonary vein triggers can often be the reason for AFib recurrence. These areas include the left atrial appendage, the left atrial posterior wall, the superior vena cava, the coronary sinus, the vein of Marshall, and the crista terminalis. If these foci are not looked for, they will not be found. They may not be inducible in the first procedure due to the overlying presence of pulmonary vein triggers. The greatest challenge in treating these foci is the limited tools we have available to induce, map, ablate, and reevaluate. Moreover, these foci may not be inducible when a patient is under general anesthesia due to sympathetic and parasympathetic suppression. Other arrhythmias such as AV nodal reentry may also be a trigger for AFib and need to be looked for. With this in mind, many ablation technologies have been evaluated over the years. Cryoballoon ablation and radiofrequency ablation still represent the two most common approaches used. Several studies have shown similar outcomes with both approaches, but there are certain advantages and disadvantages with each. AFib ablation is not a one size fits all. Treatment must be individualized to the patient - both in technology and technique. The classic EKG appearance includes a sawtooth pattern in leads II, III, and AVF; upright in V1 and negative in V6. The consequences of aflutter are similar to AFib: stroke, heart failure, and reduced quality of life. ... Treatment includes cardioversion, drugs, and catheter ablation. Ablation has a success rate of almost 95% with a low complication rate. Most atrial flutter rhythms originate from the right atrial cavotricuspid isthmus, an area of tissue between the tricuspid valve and inferior vena cava vein. And just like that, a diagnosis is made! #Cardiologists and #Electrophysiologists select the type of #heart rhythm monitor to diagnose cardiac #arrhythmias based on the frequency of #symptoms and need to detect silent #AFib or #bradyarrhythmias such as AV block or sinus pauses. Holter: daily symptoms... Event recorder: symptoms during a 4 week period, silent arrhythmia detection not needed Telemetry monitor: symptoms during a 4 week period, silent arrhythmia detection needed Patch monitor: symptoms during a 2 week period, silent arrhythmia detection needed, ease of use and no cell phone signal needed, waterproof Implantable loop monitor: symptoms are erratic and may not occur for months at a time, battery good for 3 years, wireless Apple Watch or Kardia Mobile: symptoms are erratic and have not occurred while wearing conventional monitors, auto detection of silent arrhythmias not available (yet). Holters are also used for assessment of rate control in persistent #AFib and calculation of #PrematureVentricularContraction burden or percentage. Choose the monitor for the right reasons and it will yield the greatest yield of answers to the questions you and your doctor may have. The key is correlating symptoms to rhythm. If palpitations occur during normal rhythm, that helps rule out arrhythmias. Let me explain The electrical signature of the heartbeat is the P wave, QRS, and T wave. The time in milliseconds between these signals represents the speed of electrical activation and recovery. Electrical events precede mechanical events. The P wave is atrial activation. If this is inverted, it can indicate an ectopic source of the heartbeat, or activation of the atria in reverse direction such as a junctional rhythm or supraventricular tachycardia. A wide P wa...ve can indicate interatrial conduction delay or disease. The PR interval is the timing from atria to ventricles. Slowing or block of this interval can indicate problems in the AV node or below the AV node in the bundle of His or bundle branches. If there is 2:1 AV block, and the QRS is wide, the level of block is usually below the node. If the QRS is narrow it is usually in the compact AV node. AV nodal block improves with exercise, infranodal block worsens with exercise. A delta wave or ventricular preexcitation can indicates an accessory pathway. The polarity in different portions of the ekg can indicate the location of the pathway - right bundle morphology indicates left sided pathway and vice versa. The QRS is electrical activation of the ventricles. Bundle branch block can be fixed due to scarring or functional due to a relative difference in refractory periods that occurs during different heart rates (rate dependent). Left bundle branch block carries an increased risk of structural heart disease and mortality. The ST segment is often indicative of coronary blood flow impairment such as ischemia or myocardial injury. The QT interval is electrical repolarization or recovery. Prolongation can result in torsades VT. It can be congenital or acquired. Faster heart rate shorten the QT and slower lengthen it.
