In 2018 the American Cancer Society recommended that colorectal cancer screening begin at age 45 due to the increasing rate of young-onset colorectal cancer. In October 2020, the United States Preventative Services Task Force followed to extend this same recommendation. This is big, since most insurance companies follow the guidelines set by the USPSTF. What does this mean for you? 1. If you are 45 or older and average risk, ie no family history or personal history of symptom...s, talk to your doctor about colorectal cancer screening this year. 2. If you are younger and healthy, start learning about your family history. Your doctors may recommend a colonoscopy at a younger age if you have a family history of polyps or cancer. 3. Cologuard is a stool testing kit for detection of colorectal cancer. You probably saw their commercial during #superbowl and thought . The cute character is catchy and it’s true that the test has a high cancer detection rate. But they leave out a key fact. They don’t tell you that only 42% of advanced precancerous polyps are detected with this test. More and more people are asking for stool testing for colon cancer screening, especially during the pandemic. When used appropr...iately Cologuard is a decent test to rule out colon cancer. But It does not compare to colonoscopy, which finds the smallest polyps, and removes them so the don’t become cancer. Nobody wants a test that tells you you have cancer. Also, the scare when you have a positive test - but it’s a false negative. That wait to get a colonoscopy to confirm the results is torture for patients. Our front line providers appreciate that ....... But we are starting to see the other side. The side I feared when #stayathome started. Patients are starting to show up in extremis. Hours and days late coming in with #noncovid problems Surgical problems with trajectories far worse than normal because as good citizens we are all following the rules. Maybe a little too much I can deal with this abdominal pain at home. A little too much I won’t bother my doctors. Last week, I saw a patient hit the ICU due to dehydration and kidney failure because she tried to do it alone at home after surgery. Never called. Didn’t want to bother her docs. Until she was in organ failure. This week, more surgical complications of stay at home. Conditions that could be treated with antibiotics are progressing to surgical problems as they go untreated. Please guys, #callyourdoctorfirst Most doctors offices are open and taking phone calls. Most will triage you over the phone and get you seen if needed or treated via #telehealth Many conditions can be handled without you going to the ER, especially if you reach out early. We are here for you. We don’t want you to deal with your health issues alone at home. We can often tell based on your symptoms whether you can manage at home, need to be seen, or need to go to urgent care or ER. My office is scheduling same day urgent consults.
