A 27-year-old patient came to us in need of corrective work. She previously received an implant and bone graft on a congenitally missing tooth (#7) and trusted her last dentist to place a veneer on her peg lateral tooth (#10). We diagnosed her with the following: -Gum recession induced by poor surgical execution... -Encapsulated bone graft apical to the implant apex. This occurs when a bone graft is not done correctly and becomes fibrously encapsulated. Fibrously encapsulated grafts halt degradation leading to poor bone turnover and graft failure. -Poorly fitted and contoured temporary restorations -Altered Passive Eruption: A condition which results in excessive gum overlapping the enamel limits, causing the teeth to appear shorter Procedure: -Correct the recession on tooth #9 with CAF (Coronally Advanced Flap) + a connective tissue graft. -Remove the encapsulated bone graft to reduce inflammation and pain on palpation -Harmonize the gingival zenith with an Esthetic Crown Lengthening Procedure. This is done to expose more of the underlying tooth structure in preparation for permanent restorations. We then repositioned the tissue at the proper Cementoenamel Junction (CEL) for an even gum line. We successfully provided properly fitted and contoured restorations, and temporaries designed to help the tissue conditioning. We properly designed and fitted temporary restorations that helped for the tissue conditioning. This prepared the framework for a more aesthetic outcome by the referral dentist. This patient did not want to go a day without his teeth so we restored his smile with the All-on-6 "teeth in a day" treatment. We extracted his remaining lower teeth as a result of poor prognosis and placed 6 immediate implants. This patient came in with an inflamed lateral incisor as a result of a biological width invasion due to a class 5 composite that was extending below the gingiva and restricting the biological width. This created a chronic level of inflammation that is not healthy and affects the aesthetics of the patient's smile. We performed a Crown Lengthening Procedure along with osseous surgery with tissue scalloping to harmonize the smile line, remove the overhang restoration, and re-establish the patient biological width. The after image taken 1-year post-treatment shows that the tissue health remained stable and the aesthetics are beautifully maintained.
