Let’s get down to the drain basics. Following surgery, fluid typically serous fluid composed of blood plasma from ruptured blood vessels and inflammatory fluid produced by injured cells tends to build up in the area between the skin and muscle, known as dead space. This space is to blame for the fluid collection that triggers seromas, so in a traditional tummy tuck and liposuction steps must be taken to remove fluid accumulation from that area. A tube placed in the s...oft tissue tunnel extends to the dead space, allowing you to flush away the fluid. Typically, Placement of a drainage system is carried out to prevent postoperative infection, hematoma, or seroma. In most cases, they’re integral to a proper post-op plan and help speed up recovery times so you can be back to your best self and proud to show off your PostOp results. At the end of your procedure your plastic surgeon inserts the drains usually clear tubes made of silicone through a short incision behind the primary incision. In most cases, the drains extend from either a small gap in the lower abdominal incision or the large surface area between the pubic region and ribcage. The tube then leads out of your body into a small bulb about the size of a lemon. Periodically, fluid accumulation must be emptied by squeezing the bulb, which creates a gentle suction that pulls fluid from the body. How long drains remain in place varies from person to person. Most surgeons say they usually remove the final drain within one week, but sometimes patients need to keep them in for as long as two weeks. 2 weeks. Don’t be in such a rush to remove them!!! After surgery, there is continued oozing and shedding of cells and bodily fluids at the surgical site. The drain removes fluid and this removal of fluid speeds healing. Drains are placed in wounds to evacuate fluid that would otherwise accumulate due to dead space, inflammation, infection, and/or necrosis. Fluid accumulation in a wound can be detrimental because it: Acts as a medium for bacteria... Separates tissues that need to heal together Creates pressure that can cause pain and decrease local blood flow. How does the drain work? Your drain will automatically suction fluid out when the bulb is compressed. The bulb has to be compressed very well and the drain tab has to be closed in order for the suction to work. When the bulb can maintain its compressed shape, it is a sign that suction is in effect. The color of the drainage changes from red to a yellow over time. There may be red stringy material in the drainage. These are not blood clotsDO NOT WORRY! This material does, however, tend to block the tubing. You may need to "milk" or strip the tubing. To do this, use either alcohol wipes (wrap the wipe around the tubing) pinch and pull gently on the tubing (flattening the tube). This will move the stingy material down the tube and prevent clots from forming. In some cases it may be necessary to use a syringe to suction out the build up of clotted material in the tubing. How long will drains remain in place? This varies by case. Each persons recovery differs based off various factors. Typically drains will remain in place for as little as 8 days and as long as 2 weeks. Don’t be in such a rush to remove them!!! Preparing in ADVANCE for the appropriate after care treatments and products will facilitate your healing process and optimize your results. You may notice a hardness or lumpiness to the areas treated with liposuction especially in the abdominal area & flanks, even more ...so if combined with other body contouring procedures (such as fat transfers for what is popularly known as the BBL: Brazilian Butt Lift). Normal and to be expected following Liposuction procedure. Post-surgical lymphedema (swelling) is caused by inflammation and trauma from the cannula (instrument that sucks out the fat) moving under the skin. Channels are formed by the cannula that can fill up with fluid and the tissue also becomes swollen. Manual Lymphatic Drainage helps to move the fluid by gently pumping it back into the lymph vessels. Reducing the swelling can reduce discomfort. Without Lymph Massages the inflammation can evolve into fibrosis (a permanent hardening of the tissue) or a seroma (pocket of serous fluid). I can’t stress enough about THE IMPORTANCE OF BEFORE AND AFTER PHOTOS for Cosmetic Surgery patients. Please do yourself a favor and take photographs of yourself (preferably undressed) and depending on your procedure with a BLACK sports bra and BLACK thong. There are several, very important reasons and purposes for these photos: 1 It documents your starting point, so you can compare later. Sometimes it can be hard to gauge results without having before and after photos. Whe...n you have the two images to compare side by side, the changes are much more apparent. 2 It enables your doctor to educate you about the procedure and anticipated results better. No two people are alike, so models and sketches don’t always accurately represent your personal anatomy. Photos serve as a valuable aid when your doctor is explaining how a procedure will work and what changes you can expect to see. 3 When YOU allow the doctor or a PostOp Care Specialist such as myself to share them, they can help educate others and showcase the doctor’s skill. It’s highly likely that you examined photos of your doctor’s actual patients before you began treatment. While Instagram, review websites (Yelp) and personal recommendations may help guide your decision in choosing a doctor, nothing demonstrates his/her capabilities quite like photographs do. 4 Filing a complaint or lawsuit Proving your case/complaint will be much easier when they can see visual evidence of why you are dissatisfied with the outcome of your procedure(s). Photographs provide you with the visual evidence you would require in order to be able to process your claim/complaint. 5 What better way to track your progress than to have photographs to put side-by-side in chronological order. This is a great way to watch the slow transformation and to track your healing process. The most common statement from clients... Planning and scheduling PostOp massages can help speed up recovery time and improve your cosmetic results. What is the fluid that is draining from the incision site or into drain The tumescent technique for liposuction uses a large volume of a dilute solution of lidocaine and epinephrine. The dilute tumescent fluid contains both lidocaine for local anesthesia and dilute epinephrine that shrinks capillaries. After liposuction, a large volume of blood-tinged tumescent anesthetic solution remaining trapped under the skin may slow postoperative recovery if not properly drained &... compressed. OPEN INCISIONS (no sutures aka stitches) This method provides improved patient comfort, shortens the healing time, and decreases the number of postoperative visits to the surgeon. Open-drainage with bimodal compression is a modern technique that greatly reduces the degree of pain, swelling, bruising and convalescence time. Traditional After-Liposuction Care Methods rely on 1) the closure of incisions with sutures with/without drain and 2) the prolonged use of high compression elastic garments for two weeks or more. Most liposuction surgeons would agree that these traditional methods after liposuction leave much room for improvement. The common undesirable characteristics of the healing phase after liposuction include protracted swelling, bruising, tenderness, as well as time-consuming post-operative follow-up visits with the surgeon. Minimizing swelling & Bruising: Temporary injury to lymphatic capillaries as a result of liposuction cannot be avoided. However the persistence of blood-tinged tumescent solution trapped under the skin can be prevented with REGULAR POSTOP MANUAL LYMPHATIC DRAINAGE MASSAGES & PROPER USE OF COMPRESSION GARMENTS!! many patients have expressed fear about lymphatic massage to being ‘excruciatingly painful’.
