Thinking Outside the Box One of the principles of implantology is to place implants fully in the bone. When the top of the ridge is narrow and the length is sufficient, you could perform bone reduction and get sufficient width at the bottom of the alveolar bone.... But when there’s only one site that needs bone reduction, it will make the bone level uneven and challenging to restore. This is when we need to think outside the box by: 1) designing the implant close to the top of the ridge, 2) intentionally expose partial implant at the buccal, 3) bone-graft to cover the exposed surface, and 4) choose a longer implant to reach the bottom of the alveolar bone to gain primary stability. In addition, because the AOX case is splinted together, it will provide more stability as one-piece and keep the grafted site stable.
