Advanced Micro-Endodontics

Endodontic retx of tooth no 7. Patient presented with a labial sinus tract associated with the tooth. First appointment, removed gutta percha and irrigated the canal copiously with 6% hypo. Tooth kept on oozing. Tried filling canal with calcium hydroxide. The oozing would not allow it. Left a microsuction in the canal and sat the patient up to facilitate drainage. This went on for 20 minutes but once again the tooth kept on draining. Decision was made to leave the tooth open.... Patient returned 1.5 months later. She reported that all her symptoms had gone away. On exam, labial sinus tract was no longer present. PA radiographs showed the large PARL had gotten smaller even though the tooth was left open. Canal was copiously irrigated again with 6% hypo. No oozing and drainage noted. MTA placed and canal obturated. Patient returned for a 3-month-post-op. She reported no symptoms and was very happy. Radiographs taken and compared side by side to pre-op radiographs that showed very nice healing of initial large PARL. Endo retx of tooth no 2. RCT was done originally by an endodontist. A year ago her dentist crowned the tooth. A few weeks ago patient started to have pain. She developed a swelling above the tooth and could not chew on the tooth. Today, endo access was made through a porcelain crown and it was noted that the crown was placed over the Cavit along with a sponge in the pulp chamber. Pulp chamber was also filled with fluids. Endo retx of tooth no 13. Endo access. Removed plastic obturators using my technique as described before. Remaining gutta percha removed using chloroform. Copious Irrigation with full strength hypo. Instrumentation performed. Drainage and oozing noted. Large apical diameters. Tried packing canals with calcium hydroxide but the oozing would not allow it. Closed tooth with Cavit. One month later patient returned. Accessed tooth. Instrumentation and copious irrigation done. Note...d purulence again. This time left tooth open. Patient returned a month later. No oozing or drainage noted. Canals irrigated copiously with hypo again and both canals obturated entirely with WMTA. Tooth temporized and patient was told to see his dentist for the permanent restoration. Eleven months later patient returned for RCT of a different tooth. Radiographs were taken of tooth no 13 showed great healing of the PARL.
