To our Primecare patients: Primecare informed TVFM that they would be sending the enclosed letter to all patients. If you have not received a letter, please contact Primecare to ensure that you were switched to Dr. Feeney.

To my dear patients at TVFM: I appreciate the trust you have shown in allowing me to provide for your medical needs for the past 29 years. At this time, I am writing to announce the closure of my office-based medical practice. I am no longer able to employ adequate staff to meet your needs. Even though this location is closing, Dr. Feeney will continue to be available to take care of your healthcare needs, and is available to be your primary care physician, if he is not al...ready. Dr. Feeney is relocating to 27699 Jefferson Avenue, Suite 204, Temecula, CA 92590. Phone 951-695-4688, Fax 951-695-4689. You may make an appointment by calling that office. If you are a PrimeCare patient assigned to Dr. Boggeln, please contact your health plan immediately and request to change your primary care physician to Dr. John Feeney effective 1/1/17. It is VERY important that you call as soon as possible to make this change, requesting a retro effective date to avoid an interruption in your coverage. Please fill a transfer of medical records authorization form and return it to my office no later than January 31st. The office will be open until then for patients to pick up medical records. We will continue to be available to refill prescriptions and provide referrals to primary care doctors or specialists, until the end of the month, at which time Dr. Feeney will take over that function at the above address. Your medical records will be transferred to Dr. Feeney at that time, and may be obtained by calling his new office. My desire is that the closure of my office practice be as easy on you as possible. I apologize for the abrupt nature of this announcement, and for any inconvenience it may cause you. I will continue to provide hospice and palliative care consultations in the hospitals. I hope to provide for these consultations in the outpatient setting, at home or in a facility very shortly (i.e. house calls).
