Dear Patients: As we near the end of this year, appointment slots are filling up quickly as we must space out appointment times for COVID-19 safety precautions. If you need an eye exam before the end of this year, please call in soon at 650-917-1342 so we can schedule you in. We currently pre-screen all patients before appointment times, and will require you to wear a mask and maintain a 6 feet distance from staff and guests whenever possible. If you require services to purc...hase glasses/contacts or repairs/adjustments, please also call ahead so we can best arrange a time for you so that we do not have too many people in the office at a time. June 6, 2020 Dear Patients, Starting next week on June 9th, we will resume our normal business hours. We are open Tuesday through Friday from 10 am to 6:30 pm, Saturday 10 am to 5 pm. We strongly urge appointments even for pick-ups, repairs, and browsing our dispensary so that we can limit the number of persons in our office. We will still be requiring all persons entering to wear a face mask, maintain proper 6 feet social distancing in the office, and we will pre-screen all ...persons coming in with an appointment. Patients pre-screened before appointments must confirm they: o are not currently sick or have an immediate family member who is sick or has flu-like symptoms o have not traveled in the past 14 days o have not tested positive for COVID-19 or been in contact with someone who has tested positive for COVID-19 Patients who do not pass the screening questionnaire will be asked to reschedule. New patients to our office will be requested to fill out history forms before the appointment time. We will send a copy via email please print it out and scan back to us or bring it with you at the time of your visit. All equipment, furniture, and shared surfaces will be thoroughly cleaned according to OSHA and CDC recommendations before and between every patient encounter. May 2, 2020 Dear Patients, Due to the extension of the shelter-in-place ordinance, Standard Optometry will be limiting business operations until at least May 31, 2020 to help prevent the spread of COVID-19. Santa Clara County has allowed for routine eye exams as of April 29, so we are carefully planning a slow reopening for the safety of our patients and staff. ... Dr. Lo will be seeing limited appointments for the month of May based on urgency. We will be implementing the following office protocols effective immediately for the month of May: Appointments for examinations or glasses/contacts must be made in advance as we will be open limited hours and by appointment only so that we can limit the number of people in the office at a time. Pick-up of frames/contact lenses or frame adjustments are also required to have a scheduled time. Please call and leave a message at 650-917-1342 or email us at [email protected] and we will reply as soon as possible. Patients and accompanying guardians must wear a face mask for the duration of your time in our office. If you do not have one, we will provide one for you. Only the patient is allowed in the office for the appointment, except in the case of minors 18 years and younger. Minors can be accompanied by one legal guardian. We will send a copy via email please print it out and scan back to us or bring it with you at the time of your visit. Please maintain at least a 6 feet distance from other patients and/or staff as much as possible during your visit. We have demarcated places to wait to be helped. If you are in urgent need for glasses and are currently a patient at our office, we can email you a copy of your prescription via a secure web portal, or we can try to place an order for a previously ordered frame/lenses on file as best as we can. For issues with existing orders, please call or email us to arrange a time for adjustments or to address issues; if they can be delayed, we advise scheduling these appointments after May 3, 2020. Dear Patients: We wanted to update you on recent steps we’ve taken in response to continually evolving Coronavirus (COVID-19) pandemic: 1. We are messaging all patients ahead of time to cancel and reschedule non-urgent appointments if you or anyone in your immediate family answers yes to any of the following questions:... a. Do you or anyone in your immediate family have a fever of 100 or greater? b. Do you or anyone in your immediate family have a respiratory infection, cough, or shortness of breath? c. Have you or anyone in your immediate family had contact with a confirmed case of COVID-19? d. Have you or anyone in your immediate family traveled to a high impact area In the past 2-4 weeks? 2. We are limiting the number of patients scheduled each day in our office so that we can allow for social distancing space between patient families. Thank you for your understanding if we may need to adjust your appointment time. 3. We have enacted even more frequent cleaning and disinfecting standards throughout our office, above and beyond normal OSHA standards. We are requesting hand washing before pretesting, frequently washing and cleaning commonly used items (pens, door handles etc), and cleaning of frames that have been tried on in our office.
