This patient presented to the office with an ailing implant at site 10. The patient was currently in a provisional crown. The surgeon who had placed the implant recommended that the implant be restored but it was clear that the esthetics would have been severely compromised. The decision was made to remove the implant, and a bone graft was performed, with a titanium mesh placed over the bone to obtain vertical augmentation. This is third and final part of our series this week. . 4 months later the final case was restored with a new material, Trilor, on the upper arch, and splinted crowns on 19 and 20. Trilor has the advantages of both a hybrid denture and a zirconia restoration. It is highly polishable and esthetic as well as strong, but it is lighter, intra-orally repairable, and it sounds like natural teeth when biting. This is a 1-year follow-up to an extraction of an impacted 17. The root of the tooth was pressing against the IA nerve, which is why the previous dentist never removed the tooth. The infection around the tooth became so large that the patient was susceptible to a jaw fracture. This patient had a very large multi-abutment bridge from 4 to 12. Teeth 8 and 10 developed a new periapical infection, and I suspected a vertical root fracture. The bridge was sectioned mesial to 7 and 11, and teeth 8 and 10 were extracted. The patient had large buccal defects, but a bridge of bone coronal to the defect. I utilized this bone and the palatal bone to place immediate implants at sites 8 and 10 and delivered an essex appliance. This patient had an old failing implant at site 9. It was starting to affect 8 and 10. Significant bone loss is seen around the implant, especially at the mesial of 10. There was no way to salvage the implant so the decision was made to remove it. The implant was torqued out without removing any additional bone. It took over 250 newtons to remove the implant. The site was grafted with a combo of demineralized and mineralized bone, and the bone was covered with a titanium mesh... and tacked into place. The patient may or may need a connective tissue graft at the time of reimplantation. This patient presented to the office with bone loss on one of her implants. About 10 years ago she had implants placed at 18 and 19. Surprisingly, there was zero bone loss at 18, but about 40% to 50% at 19. I suspected foreign body irritation from excess cement, especially with this particular implant, which is a tissue-level Straumann implant. During the surgery I did not see any cement, and was unusure about what had caused this defect to be such an outlier. Care was taken ...to remove the granulation tissue, disinfect the implant, graft around the implant and place a membrane.
