Immediate implants are commonplace in our practice. Single rooted teeth can be great candidates due to the structure of the surrounding bone, but molar sites often work well too. If there is good stability and the crestal bone and surrounding soft tissue is adequate, we'll often fabricate a provisional. This particular one was made chairside out of acrylic, but we also use a io-scanner and have our lab use the digital impression to make the provisional. Autogenous connective tissue was used to treat this infected implant at #6. Patient presented with heavy exudate and bleeding on the straight facial with a 6 mm pocket. A CBCT showed minimal bone loss. ... Flap access was used because we wanted to directly inspect and clean the implant. You can see slight loss of the distal papilla. Also note that the healed tissue is still darker. Hyperpigmentation is a common side effect of chronic inflammation and is very difficult to treat. Grafting to cover recession with existing restorations? Sure! It can be done, but best with a team approach. Problem:... As you can see in the circle, the original CEJ of #6 is no longer present. You'll also notice severe recession and bone loss. (Miller 3). Since it is not possible to graft over composite and expect a healthy pocket, the restoration needs to be removed. The patient (and the dental team) desires an esthetic result. There also is marginal decay. Solution: 1. New restoration 2. Ginigival graft to cover remaining defect The restorative doctor crafted a restoration with an artificial CEJ in balance with the patient's smile. Since the gum is still very far away, they were able to polish the portion which will be in contact with the future graft. Wear and tear? Root wear and bone loss? Gingival grafting may fix both. A biologic solution for a biologic problem. When there is heavy root wear or NCCLs, we work with the patient's restorative doctor to decide the best approach. Some patient's benefit from restorative options alone. Others, work well with grafting (connective tissue here). Many severe defects are best suited for a combined approach.
