Trendelenburg’s sign or pelvic drop occurs in individuals with weak or paralyzed ...hip abductor muscles (gluteus medius/gluteus minimus). Damage to the superior gluteal nerve is often found in patients with positive signs, as this nerve innervates these muscles. The major complication of this issue is that the gait cycle is abnormal in patients with a positive Trendelenburg’s sign. In a normal individual, during the stance phase of the gait cycle (which accounts for 60% of the total gait cycle), the gluteus medius works to maintain both hips at the same level. However, during the stance phase of the affected limb, the weakened abductor muscles allow the pelvis to inferiorly tilt over the swing limb. The patient will compensate for the pelvic drop by lurching the trunk to the affected side to maintain the level of the pelvis throughout the gait cycle.
