Saint Agnes Medical Center's chronic disease self-management program, A Healthier You, has improved the lives of hundreds of community members living with chronic health conditions. The six-week program is free of charge to any community member with, or at risk of developing, a chronic disease such as diabetes, arthritis, or hypertension, among others.
