Canal Outreach Project COVID 19 Feedback Survey We are with a partnership between Marin County, City of San Rafael, Kaiser Permanente, and Canal Alliance. We are inviting you to take a quick survey regarding COVID-19. We are asking questions to understand how to better support people living in the Canal District. The survey is voluntary and we will not ask your name so your answers will be anonymous. While you will not receive anything for taking the survey, your answer...s will help us better understand the needs of this community. The survey should take about 10 minutes.
