February is National Children’s Oral Health Month! One of the most common questions we get asked is when to start flossing those baby . We recommend around age two when the primary molars come in and the primary teeth start to get closer together. Even though there may be spacing present, it’s a good habit to instill at a young age so when that two year old goes to college, knows to floss once a day! Parents and caregivers can use floss picks or regular string ...floss for small children. A tiny bit of composite resin can make a BIG difference! This patient had a chief concern of sensitivity where the tooth was chipped. The white spots and slight spacing between the centrals were a long time concern, but the patient never had a reason to address it before. Conservative resin bonding with minimal removal of tooth structure was used here to repair the chip, and also restore a more natural appearance and shade to the central incisors. Watch or treat part 2: Take a look at tooth #20. Would you restore the distal? Let’s talk about cracks vs craze lines! A crack extends through the enamel layer of the tooth and into the dentin, the more porous inner layer of the tooth. Bacteria travel through the cracks straight into the dentin and create cavities that are difficult to visualize on an x-ray, so a thorough clinical exam is imperative. Small cracks with decay, such as this one on #20, are usually restored ...with conservative bonded fillings. Teeth with larger cracks may need an onlay or crown to restore the tooth. Craze lines are superficial cracks in the enamel surface only - like a stress fracture on the tooth. No restorations are indicated here. Cracks and craze lines can signify excessive pressure on the teeth from parafunctional habits like grinding or clenching AKA #bruxism. To prevent further progression of cracks and craze lines, we prescribe custom made nightguards to protect the teeth during sleep when most harmful clenching/grinding takes place. @ Potrero Hill
To watch or to treat? We see patients who have had cleanings and check ups within the last 6 months who have deep caries lesions in teeth that were being monitored. Or, a patient will return to a previous dentist or see another dentist after hearing our treatment plan, and the second opinion is that the fillings should not have been done or treatment planned at all. We spend 90 minutes with a new patient to educate with intraoral photos and take clinical photos before/during/after procedures. We are ethical and honest dentists that just want to help our patients, and we believe that most people in our profession are too. So, do any other dentists out there share this dilemma? Making teeth look like teeth again! These old silver fillings were deteriorated and leaking, causing decay under the fillings and cracking of the tooth (swipe to see fracture).
