Earlier this week I worked with a new client who had pain in his left wrist on the ulnar side while gymming since a few months. His pain increased in end range ...extension, or while doing pushups or a bench press. The intensity worsened overtime and he was advised to take cortisone injections. The pain reduced with injections but was still persistent while gymming. During palpation we found significant tightness and tenderness along the wrist extensors, especially the Extensor carpi ulnaris. We tried a few push ups to check his form, and his left scapula and shoulder were completely off. Using Neurokinetic Therapy protocol, we were able to identify that his Extensor carpi ulnaris was facilitated for his latissimus dorsi which was causing shoulder dysfunction during push ups. The wrist is at risk of injury due to improper mechanics/repetitive overload while weight lifting. Scapular and shoulder dysfunctions are one of the major contributing factors in wrist injuries. Homework was to release the Extensor carpi Ulnaris followed by Lat activation exercise. He was also taught to correct his form while push ups/ bench press to avoid excessive ulnar deviation during these movements. This will help to reinforce the new pattern and optimise normal function. New client, tore her pectoralis major weightlifting, and now has shoulder, neck, and jaw pain. She wondered if those were related. That always makes me smile. N...KT testing found the pec minor overworking and the pec sternal underworking. Released the pec minor at the coracoid process, where she was hurting, and then activated the pec sternal. Then NKT testing found the upper trap and masseter overworking and the pec sternal underworking. Released the upper trap and masseter, then activated the pec sternal. Now her jaw, neck, and shoulder all felt better and her pec stronger. See more

My client was puttering around the house, tripped, and landed with his left shoulder blade against a light switch. Now he has pain in that area of impact and hi...s arm isn't functioning too well. He pointed to where the infraspinatus and posterior deltoid cross. NKT testing found the infraspinatus uninvolved but the posterior delt was. The upper trap was overworking and the post delt underworking. Released the trap and activated the delt. That helped some but there was still discomfort. Then testing found the post delt overworking and the lat underworking. Released the delt and activated the lat. Now his shoulder felt much better. Can a muscle like the posterior deltoid underwork in one relationship and overwork in another. You bet it can! Different movement patterns require different fibers and sensory nerves of a muscle to participate. That can change its neurological state. See more

June 19, 2020, the State of California and Solano County Health Officials gave the approval for Massage Therapists to re-open their centers. Evelyn Olivas, Olivas Therapeutic/Sports Bodywork’s Principal Massage Therapist, is moving forward with caution She understands the client’s health security comes first. Here are the general guidelines for mandated by the State of California and Solano Country (If you want the detailed State and Local Guidelines please e-mail: [email protected]): 1. Worksite Specific Plan 2. Topics for Employee Training 3. Individual Control Measures and Screening 4. Cleaning and Disinfecting Protocols 5. Physical Distancing Guidelines *Additional Considerations for Massage Services (Non-Healthcare Settings) a) Ask clients to wash their hands before any services are provided. b) Workers must wear face coverings at all times and should consider wearing a face shield for eye protection (with a face covering), if available. c) Customers must wear face coverings during the entirety of the massage service. d) Consider alterations to the treatment table set-up to support the required cleaning and disinfecting protocols. This could include using disposable face cradle covers and/or protecting the table, table warmers, bolsters, and other items with washable barriers like pillowcases that can be removed and replaced between each client. Barriers are not a substitution for the required cleaning and disinfecting protocols. e) Evaluate whether facial massages or other hands-on work to the face will be offered. If providing such services, use non-latex gloves for this part of the treatment. Facial massages should not be performed if it requires removal of the client’s face covering. f) Provide any hand treatments as the last part of the service. Shout out to a client who is a track coach and PT, a nice review of a topic we cover in depth in our classes as part of the "Too Much / Too Soon" process of musculoskeletal issues MEDIAL / TIBIALIS ANTERIOR SHIN SPLINTS Notice the very comprehensive rehab protocol this and Anterior Shin Splints involve. Certainly not a condition one 15 minute quick fix massage session can eliminate despite what some massage teachers try to sell us. TIBIALIS POSTERIOR TENDONOPATHY Tibialis posterior (TP) tendonitis is an overuse injury causing inflammation of the TP tendon. Tendonopathy is probably a more accurate term to use as it refers to wear and tear or degeneration of the tendon rather than acute inflammation. The TP passes down the back of the leg and under the medial malleolus. It inserts on the lower inner surfaces of the navicular and cuneiform bones in the midfoot and the base of the 2nd, 3rd, 4th and fifth long metatarsal bones under the foot. It is used to plantarflex the foot and to invert the foot. Causes Athletes who overpronate are at increased risk as this places more strain on the TP causing it to repeatedly overstretch and overwork more. Sports that increase the risk of TP tendonopathy are those which require prolonged stretching of the TP like ballet dancing, Ice skating or sprinting. Diagnose Passive eversion/resisted inversion. Heel raise on a step w/ difficulty keeping the ankle straight, remains rolled in & flattened. MRI scan. Signs & Symptoms Pain on the inside/behind the ankle with radiation along the TP tendon length. Pain comes on gradually over time. Crepitus can be felt when the tendon moves. Swelling most likely in later stages around the back of the malleolus. If a lot of swelling acute partial tear? Treatment Decrease initial pain and inflammation. Improve mobility and flexibility. Improve strength. Re-establish neural control and coordination. Return to full fitness. What can the athlete do? Cold therapy: 10 minutes every hour. After acute stage, heat may be more beneficial. Compression bandages. Stretching exercises for TP & calf muscles if not painful. Specific strengthening exercises for the TP. What can a therapist do? Electrotherapy Anti-inflammatory medication. Deep tissue treatment. Rehab program to strengthen the TP with focus on developing eccentric strength. Exercises can begin as soon as they can be performed without pain. Orthotic inserts can be prescribed & fitted if required to correct poor foot biomechanics. If the tendon is ruptured then it must be repaired surgically.

Taking some time to give back to those who support us. WORKING WITH CLIENTS POST OP - SCAR TISSUE. A THOROUGH STUDY ON THIS TOPIC. BE INFORMED, ALWAYS BE LEARNING, GROWING. AND ABOVE ALL, STAY CURIOUS. Be very wary of someone trying to teach you they can "eliminate" full body lesions including adhesive scar tissue in one 15 minute massage session. Read the attached research and you make your own decisions. Just because someone says that it's true, doesn't make it true. "The fascia is the philosophy of the body, meaning each body region is connected to another, whereas osteopathy is the philosophy of medicine: the entire human body must work in harmony." "Conclusion The skin is an organ that can influence body homeostasis. It is good practice, when evaluating the clinical picture of a patient, to always ascertain the presence of scars. Looking at the patient as a system and not just as a symptom or a symptomatological segment will help the clinician, and all those professionals who use manual techniques, to find a more effective treatment. A scar can produce different symptoms, which can affect the neurological sphere, the fascial, and the visceral area. The skin surface is a means to communicate with the nervous system, to understand it, and to give therapeutic information. We can conclude by affirming that the skin and the fascias represent the skeletal system of the nervous system. General organization and territories of innervation of the sympathetic nervous system (blue) and parasympathetic nervous system (red). We can logically assume that in the presence of a scar, these receptors may experience an alteration, resulting in transmitting nonphysiological signals and creating a pathological reflex arc. The structures of the spinal cord and blood vessels, rear projection, are made up of various layers but are continuous. However, it is usually ignored when the tissue where the tension is observable is in an unbalanced condition, as in the case when a scar is present, the cells cannot properly interpret the message, giving consequent anomalous responses."

Taking some time to give back to those who support us. Now seems like good time to review our old friend THE BRACHIAL PLEXUS - What we call in class, Upstream effects Downstream. From my files. I like these illustrations. "This is the brachial plexus, a network (knitwork) of nerves that supplies the upper extremities divided into roots, trunks, divisions, cords, and terminal branches. When I was in anatomy lab, I remember the brachial plexus being impossibly difficult to memorize (despite frank knetter’s iconic rendition) and how helpful a physical model would have been. I had ample time to experiment with I-cords, testing out various ways of connecting and branching them. And then, many awkward stares and ninety five hand-stitched letters later, I completed my anatomically correct (albeit simplified) brachial plexus! I can’t say that it’s the most aesthetically pleasing project (pretty sure that yellow is red heart super saver), but it’s probably one of the nerdiest things I’ve ever done (except for third grade, when i voluntarily skipped recess to help sort mail)."
