Patients often ask me what they will look like immediately after surgery. Will they have to wear a patch? Will the eyelids be swollen shut? Will they have a visible scar? This is one of my male patients immediately after upper and lower eyelid blepharoplasty. Antibiotic ointment is placed over the incisions. The upper eyelid incision is hidden within the eyelid crease (fold). The lower eyelid incisions are made from the inside of the eyelid and are thus not visible. My patien...ts typically do not need to wear a patch or have adhesive strips over their eyelid incisions. Swelling and bruising often worsen over the following 48 hours before regressing. SUBCONJUNCTIVAL PROLAPSE OF ORBITAL FAT Fat from behind the eye (orbital fat) can herniate and prolapse forward into the space underneath the surface lining of the eye (subconjunctival space). This occurs when a capsule, called Tenon's capsule, is violated, either spontaneously or after trauma or surgery. Prolapsed orbital fat can appear as an elevated yellow-orange fatty mass, usually in the top outer corner of the eye. It can occur in one or both eyes. This is a benign cond...ition that can be observed without surgery. As the fat continues to prolapse, it may become visible beyond the eyelids and can cause eye irritation. Surgery can be performed to remove the prolapsed portion of the fat. Epiblepharon is a condition in which an eyelid muscle (the preseptal portion of the orbicularis oculi muscle) and an extra roll of skin push the eyelashes upwards toward the eye. It is present at birth and occurs most commonly in children who are Asian, Hispanic, or Native American. Epiblepharon is most often asymptomatic. However, in certain cases, the eyelashes can rub against the front of the eye (cornea) causing a foreign body sensation, redness, irritation, avoidance of ...light (photophobia), and tearing. Most children outgrow this condition without surgery as the facial structures develop. Occasionally, surgery is needed to correct the condition if it does not resolve on its own and if the eyelashes continue to irritate or scratch the cornea. Oculofacial Plastic Surgery Tools of the Trade: The Chalazion Clamp. The chalazion clamp is a surgical instrument that is used to aid in the incision and drainage of chalazia/styes. It was originally designed in the 19th century by French ophthalmologist, Louis-Auguste Desmarres (18101882). The clamp helps with isolating the chalazion, minimizing bleeding by applying pressure, and stabilizing the eyelid during the procedure. SURGICAL DRAINAGE OF A CHALAZION/STYE A chalazion is an inflamed bump in the eyelid that occurs when hardened oils block an oil gland. Treatment is initiated with frequent warm compresses and/or topical medications. Non-resolving or large chalazia can be treated surgically in the office by draining the inflamed lesion. This is usually performed through an incision on the inside of the eyelid, which leaves no visible scar. This video demonstrates incision and drainage of a cha...lazion. A special clamp is first placed to isolate the chalazion. The eyelid is then everted, and an incision is made on the inside of the eyelid. The contents of the chalazion are then scooped out with an instrument called a curette. Several patients and friends have asked me today about Vice President Mike Pence’s eye during the debate last night. While I cannot know for certain, it appears that Vice President Pence may have had a subconjunctival hemorrhage. What is a subconjunctival hemorrhage? A subconjunctival hemorrhage is a very common phenomenon that occurs when one of the blood vessels underneath the clear lining of the eye (conjunctiva) bursts. This causes the white of the eye to appear bright re...d. It can occur after sneezing or coughing, heavy lifting, straining, vomiting, or aggressive eye rubbing. I also see it commonly after eye (or even eyelid) surgery. A subconjunctival hemorrhage causes no pain. It may spread along the entire white of the eye, turn yellow, and then disappear within two weeks, much like a bruise. WEDGE EXCISION OF EYELID LESIONS Skin lesions and bumps on the eyelids are fairly common. They can be benign or malignant (cancerous), and can often grow in size causing eye irritation or an unsightly appearance. Lesions along the eyelid margin, which are close to the eyelashes, deserve special consideration when attempting surgical removal. An inappropriate surgical technique can lead to notching of the eyelid, loss of lashes, or misdirection of lashes that can often rub aga...inst the eye (trichiasis). To completely remove lesions along the eyelid margin, I often use a technique called "full thickness wedge excision and reconstruction," where I fully excise the lesion, bring the eyelid segments on either side together, and suture the eyelid in a way to prevent notching of the eyelid margin and to maintain the natural direction of the eyelashes. This lovely patient was bothered by the appearance of a lower eyelid benign nevus, which had slowly been growing since birth. I excised the nevus and reconstructed the eyelid. She has healed very well after her procedure. Results shown are 6 weeks after surgery.
