Fully vaccinated and still masked With all due respect to the CDC, most physicians completely disagree with the new mask recommendation. I like to call it Ivory Tower Medicine. When you are removed from the trenches and real world medicine, you tend to be blinded to the reality of the situation. I understand the science may support this decision, but other considerations weren't taken seriously. ... The reality is: People can still get sick despite being fully vaccinated with either of the shots Huge swaths of the country are still not vaccinated (kids, vaccine-hesitant) People are LYING about their vaccination status with no way to verify it Businesses will now have a much harder time enforcing their mask policy Many of us saw the light at the end of the tunnel. Now, the tunnel just got longer. I'll still be volunteering my weekends to vaccinate the most vulnerable and spending every weekday battling the vaccine misinformation. Think a melanoma diagnosis is black and white? With May being Melanoma Awareness Month, you’ll likely see everyone and their mom post about the ABCDE’s of melanoma detection and what to look out for clinically. I think you’ve got that part down. But what happens after a biopsy is taken of a suspicious mole? Is it like a blood test where a computer spits out an answer? ... Some facts: A melanoma diagnosis is made by looking through the microscope at a slice of skin less than 1/10 the thickness of a human hair A melanoma diagnosis can be very straight forward or extremely difficult depending on many different factors The patient’s age and clinical site of the lesion play a big role in diagnosing a melanoma There is some variability among those diagnosing melanoma. Ask 3 different physicians and you may get 3 different diagnoses The most reliable diagnoses come from board certified dermatopathologistsnot general pathologists or dermatologists Know where your biopsy is going. You know the person who took the biopsy but likely have no clue who is reading your slide. Get the pathology report. Get a second opinion if there is any doubt. "Mortui vivos docent" - The dead teach the living. A pathologist’s relationship with death is, well, complicated. I am lucky to straddle both the clinical and pathological realms, but most pathologists do not. After my eShadowing talk with Medical School Headquarters - The Ultimate Premed Resource ... https://youtu.be/u3swtcMDJSI I received a very interesting message from a medical student considering pathology as a residency: Surrounded by so much disease and death, where do I find humanity? Where do I think a person begins and ends? Out of the hundreds of questions I received after that talk, these truly captured me. It is easy to philosophize about humanity while holding bits of tissue or organs in your hand or cutting open a freshly deceased, still warm body. Amazingly enough, not many of my colleagues wanted to engage with me over questions like this when I pondered them in training. I suppose it is easier to focus on the tasks at hand, make the diagnosis and move on, then it is to discuss and try to draw any conclusions about the essence of humanity. It’s truediagnosing so much disease in a given day and performing autopsies on all age groups (even fetuses and babies), can get to you. But making a connection to the patient’s life is important for pathologists to do. Realizing that every slide represents a person with her/his hopes and fears pinned to your diagnosis, made me the best pathologist I could be. When I performed an autopsy, it was easy to think of the body as component parts to dissemble and formulate answers from. Death had taken the person out of context. I found their lingering humanity in the small things that remainedfingernail polish, tattoos, stomach contents of their last meal. I secretly treasured those glimpses into their life. I carry many of the cases I’ve seen over the years with me, as many pathologists and other physicians do. Covid-era conference attendee!! I loved going to conferences and am lucky to get two different fields to choose my conferences from: pathology and dermatology. Breast cancer treatment and skincare? Yes!! All too often I see patients who were given instructions from their oncologists about what to expect from the radiation and chemotherapy without mentioning much about their skin care routine. So, why do you need to change your tried and true routine up?... Chemotherapy drugs can cause a whole bunch of changes to your skin such as severe dryness, acne-like breakouts, dark spots, itchy rashes, hives...and so much more. What to do: Make sure everything you use is fragrance free Use gentle cleansers like @cerave or @cetaphilus - not harsh soaps Take short, warm showers instead of long and hot Moisturize with a thicker CREAM emollient with a humectant like hyaluronic acid as soon as you get out of the shower and pat dry. Lotions are really too thin to do any good. I love @cetaphilus cream @larocheposayusa Lipikar Balm and @officialvanicream Don't forget the sunscreen! Chemotherapy agents can make you extremely sensitive to the sun and blue light from devices, so SPF is mandatory. Specifically zinc-based formulas. Not all maskne is due to traditional acne. See a #physician to make sure you don't have another cause like perioral dermatitis, rosacea, infection, etc. Make sure your skin is clean and hydrated with a gentle moisturizer prior to putting on your mask. Avoid wearing any makeup under the mask! Especially lip products that tend to be thick and will clog pores. Be as gentle to your skin as possible. This means stopping all harsh cleansers, toners, scrubs, masks and brushes that make your skin more sensitive and weaken the barrier. Once the mask is off, use gentle cleanser and barrier repair cream on any areas of skin breakdown. I like to keep it simple with either Vaseline, @cerave Healing Ointment or @eltamdskincare Barrier Renewal Complex. If your skin is ulcerated or infected, you may need a medication. A prescription cream or oral medication may be needed to address the underlying cause of the maskne. The fabric of the mask is very important! Cotton will be better against the skin rather than synthetic materials. Use a clean one every day and wash with fragrance free detergent. As always, for any of my frontline physician and nurse colleagues working in our Orange County hospitals who are experiencing skin issues from PPE, I’m giving free telemedicine appointments to discuss prevention and treatment options.
