Today’s featured patient had a revision rhinoplasty with me nine months ago. Her prior silicone implant was causing a pollybeak appearance to her bridge, so I removed and replaced it with a diced cartilage graft with fibrin glue! Some cases are challenging and some are simple! When our rhinoplasty patient’s husband asked if I could remove this mole on his nose, I was happy to oblige. . One key to minimizing the scar is to orient the mole removal ellipse along the relaxed skin tension lines or natural lines/wrinkles on the face. Also, the skin edges need to be brought together with little tension and with the skin edges everted (slightly raised) at the beginning. Here’s an immediate before/after of my patient who had an open revision rhinoplasty with rib cartilage! This wonderful gentleman underwent a prior rhinoplasty elsewhere, which left his columella severely retracted, his tip overrotated (upturned), and his bridge crooked and irregular. He was seeking a taller, straighter bridge, a more defined tip, and a longer columella. His airway was also severely obstructed on his left side. . Because the little septum he had left was short... and twisted, I had to use rib cartilage to lengthen his columella, push the tip of his nose downward/forward, and build up his bridge.
