Elderly patient who had chest discomfort at 300 am. Due to lack of transportation did not come to the ER till mid morning when her daughter was available. EKG i...n the ER showed evidence of inferoposterolateral acute coronary ischemia. She underwent emergent cardiac catheterization that showed occluded Circumflex Coronary artery (Cx) [picture 1&2] with high grade complex lesion in Left Anterior Descending (LAD) coronary artery spanning across and into the Diagonal branch (Diag) of LAD [picture 4]. Intervention of Cx was done with thrombectomy, balloon angioplasty (POBA) and 2 bare metal stents [picture 3]. As a staged procedure LAD & Diag were addressed. LAD and Diag were wired simultaneously. Angiosculpt balloon was used to treat Diag ostium and Bare metal stent implanted in LAD [picture 5]. Patient was discharged home day after the LAD/Diag intervention. With multi vessel coronary artery disease she is fortunate to have survived this episode of acute coronary syndrome. See more

58 year old man who self referred after he was told at a county health fair that he has abdominal aortic aneurysm. CT scan as an out patient confirmed the diagn...osis. Largest diameter by CT scan was 4.6 cm. Options of open aneurysm repair v endo vascular (percutaneous) options were presented. He chose endo vascular. Picture 1 shows angiographic appearance of the aneurysmal aorta. Picture 2 shows post trimodular graft implantation and hence endo vascular repair. Patient is walking on post procedure day 1 (today) and is scheduled for discharge on post procedure day 2 (tomorrow). Just about 5-7 years ago this would have been a massive undertaking with major abdominal incisions and at least 1 week hospitalization. Patient in case is wondering where the scar is? Kudos to science and technology. See more

70 year old patient who had heart cath which showed multi vessel CAD. Left Subclavian artery showed 99% lesion at cardiac cath. Left internal mammary artery (LI...MA) was not used by the surgeon in spite of multiple discussions. The patient under went 3 vessel bypass with vein grafts (not ideal). LIMA branch of left Subclavian artery should have been used. Patient had stent of left Subclavian artery successfully today. She unfortunately missed out on LIMA. Post procedure her pressure gradient of 80mmhg between Aorta and Left Subclavian artery was abolished. See more
90 year old lady who called me to complain that there was something coming out of her shoulder. She had pacemaker implanted in 2005 and battery change in 2011. ...She was last seen by me in December 2013 and the pacemaker site was intact. The pacemaker was still functioning. She will go for laser lead removal today and new pacemaker implant in a few days. See more
