Very intelligent 9 yo girl with severe dental phobia and obesity (120 lbs, > 99% for BMI). Refused intranasal sedatives (fentanyl, precedex). Refused to sit in the chair for mask induction. In my opinion, too big for oral midazolam (besides it tastes horrible) and intra-muscular injection of ketamine risks psychological trauma. Given her age, size and intelligence I really wanted to get her cooperation for sedation. My solution? Oral valium and clonidine. Worked like magic. Patient had a smooth, gentle anesthetic induction and did well. Clonidine is a central alpha-2 agonist that can produce sedation without respiratory depression. #pediatricsedation #pediatricanesthesia #dentalsedation #dentalanesthesia #anesthesiologist #clonidine #pediatricdentistry

I was urgently called this week to provide anesthesia for an oral surgery office. The patient was over 350 lbs, and had a hx of atrial fibrillation, congestive heart failure, diabetes, gout, and sleep apnea. I provided anesthesia for a 16 yo boy for an oral surgery procedure. He had a surgical repair for a complex congenital heart condition, Tetralogy of Fallot. Sedated a young woman for oral surgery who had a heart transplant. Because the donor heart is partially denervated, some maneuvers (eg. carotid sinus massage) and medicines (eg. atropine) are ineffective in these patients. Heart transplant patients are also much more dependent on the Frank-Starling curve so preload (intravascular volume) is very important to maintain. Provided anesthesia for a young patient with sickle cell disease in the oral surgery office. These patients can develop acute chest syndrome and other problems during surgery.
