Reducing a dorsal hump is probably the most common maneuver in rhinoplasty and can seem straightforward. But making the dorsum stable and symmetrical - especially from the frontal view - is often a challenge. And then there’s the tip. We’ve got rotation and projection to consider, not to mention skin thickness and the quality of the underlying cartilages. Plus, any changes we make to the dorsal aesthetic lines necessarily affect the tip - and vice versa. W...hen we downsize the nose, it’s not simply about making a tinier tip - it’s about manipulating the light and shadows to create a more refined shape. It’s a precision game. The more surface area receiving light, the bulkier the tip will appear. Anyone who uses contour makeup understands this concept already: during rhinoplasty, I create the same balance of highlight and shadow. This patient wanted to reduce her dorsal hump and tip bulkiness. At 2 years post op, see how refined the tip looks now, especially in the frontal and oblique views? Can you spot what’s going on here in the pre-op views? FRONTAL VIEW She’s got a reverse C septal deviation, which is why her dorsum bends to her right, forming a backwards C shape. Did you also notice her twisted nasal tip?... BASAL VIEW We have a caudal septal deviation on the left side - doctorspeak for the septum partially blocking her left nostril. You can really see the tip twist from this angle as well. LATERAL VIEWS She’s got a foreshortened infratip lobule, which means her columella looks flat with no curve to it. Her tip is over-projected, extending too far compared to her other facial proportions. To straighten a deviated nose like this it all starts with getting the septum straight from top to bottom. Like any structural foundation, this is the cornerstone that we can then build off of to ensure a nose that is as straight as possible, which is always an important goal in rhinoplasty. To complete her transformation, I reduced her tip projection and improved to create more balance.
