Payroll Documentation You must provide a document that shows 12 months of payroll costs. You can use a payroll processing provider’s PPP report if you work with one already. If you don’t use a payroll provider, upload a spreadsheet showing the data for your estimated loan amount calculation. 2. Employment Tax Documents Form 941 for Q2, Q3, Q4 2019 and Q1 2020. Please make sure these IRS documents are in line with your calculated loan amount. 3. Driver's License or Valid Passport For Driver’s License, copy front and back of all owners over 20%. For Passport, copy pages 2 and 3 (pages with photo and signature). 4. Bank Statement or Voided Check Link your account or upload a voided check or bank statement as proof of your active customer account.
