@dr_carly had one last very special patient waiting for her #dentistmama to get home from a late night at the office. Losing a tooth for the first time can be a little scary but Maya took a deep breath, trusted her mama, and was so proud of herself when it finally popped out! The @getcocofloss trick works everytime! Here's your Friday reminder from your favorite dental family (now more than ever), " BE KIND ALWAYS, FOR EVERYONE YOU MEET IS FIGHTING A BATTLE YOU KNOW NOTHING ABOUT." On the surface each of these teeth simply had an old silver filling or a sealant which looked more or less okay. In most cases the patient had almost no reported symptoms or idea that there was an underlying problem. What we find more often than not is a hidden disaster waiting to happen. Multiple deep cracks... traversing the entire tooth, decay that was hidden by corroding metal, undermined tooth structure just waiting for the wrong bite to fracture irreparably. These teeth are like most people these days. Holding it together on the surface, under incredible external stresses, and one wrong move away from breaking. Let's try to have some compassion, with our teeth that have served us well all these years, and with the people in our community. The masks and distance and screens make it even harder to see people's struggle, they hide the surface indicators that something inside is not okay. But if we turn a blind eye, hiding behind our "mask" or an old silver filling, we will be attempting to avoid the inevitable, and by not addressing the problem it will only get worse and more difficult to handle in the future. I'll leave you with that and another one of my favorite dental quotes, "Be true to your teeth, or they will be false to you." Your lungs and airway depend on correct breathing, and side effects are disastrous on your health. Mouth breathing is related to signs in the mouth including:... A long narrow face A narrow mouth A high palate Crooked teeth (upper or lower) A gummy smile A tongue-tie Other signs of mouth breathing A blocked sinus Swollen tonsils & adenoids A dry mouth in the morning Sleeping with your mouth open Snoring Your nasal passages are sinuses designed to prepare air for delivery to your lungs. They act as a humidifier, warming and moistening the air. They also remove debris and act as a first-line of defense against unfriendly microbes. One of the most important ways that nasal breathing helps oxygen flow is via a gas called nitric oxide(NO). The role of nitric oxide in the body and respiration was only recently identified. Nitric oxide is produced in the nasal sinuses by specific enzymes. It’s instrumental in delivering oxygen around the body efficiently because it regulates blood flow. When it mixes with air delivered to the lung, it increases arterial oxygen tension and reduces blood pressure. Your tongue is a surprisingly good indicator of your breathing health. Proper tongue posture is crucial for palate and jaw growth and straight teeth, but it also holds your airway open. Proper tongue posture contributes to oxygen delivery and straight teeth in many ways. These include: 1. allowing nasal breathing to expand the maxilla 2. applying direct upward pressure to expand the palate 3. opening airways (due to its muscle connections to the soft palate, spine, and base of the skull) 4. activating the neuromuscular pathways that train the airway to stay open during sleep Use tongue ‘suction’ to support nasal breathing: 1) Seal the tongue entirely to the roof of the mouth 2) It includes the tip of the tongue just behind the top teeth and the BACK of the tongue, sealed up at the border of your hard and soft palate. 3) Practice this ‘suction motion’ - that strengthens the the muscles to support the airway

We are always looking at our patients as a whole person, not just a mouth or a tooth. During the initial exam I will often discuss allergies, asthma, diet and nutrition, supplements, sleep, breathing and airway, teeth grinding and acid erosion, growth and development, and I often will refer patients for consults with other specialists such as the Orthodontist, Ear Nose and Throat Dr, the Periodontist to address gum issues, a sleep specialist for a sleep study, a myofunctional therapist to address speaking and habits, and various others depending on the patient. We want to be sure we are taking the very best care of our whole patient!

Digital Dentistry from Home: A Series! We want all of our lovely patients to know that we are thinking of you and your families during this time. We hope you all stay safe and healthy and enjoy this extra time to slow down and spend time with family. When we really take time to slow down and pay attention we can learn so much about ourselves and our environment. These photos for example, tell an incredibly detailed story if you just take the time to notice. While editing and ...cropping and planning the new crown shape some other very interesting observations can be made. Notice the anterior wear patten as the patient slides his jaw right and left, this tells us he is likely grinding his teeth at night and this makes us ask why that might be? The clenching or grinding may also be contributing to the gum recession that can be observed on several teeth. When we are planning the size, shape, and material of his future crown we will need to keep in mind his bruxism habit and be sure he wears a night guard to protect his teeth going forward. We should also take note of the small gap between his two front teeth. Why is it there? Has it always been like that? Does it bother him? If we close it will it reopen with time and forces from his tounge and grinding? Digital Dentistry from Home: All of my dental colleagues, friends, and family know that the hardest esthetic feat in dentistry is replacing a single poorly matched anterior crown (often a very old restoration done as a result of trauma when the patient was much younger, meaning the remaining tooth structure usually leaves much to be desired. The remaining tooth stump is often discolored, cracked, root canal treated, or nearly nonexistent.) Matching the patients adjacent natur...al central incisor right smack at the midline of their smile is a challenge we take very seriously and gladly accept. This process takes patience and cooperation between our office, our lab, and the patient. Involving proper hygiene, bleaching, photos, trips to the lab for a custom shade and personalization, often all before ever starting the procedure. Finally, cut off the exisitng crown and reprepare the underlying tooth structure. We temporarily send the patient home in a beautifully lab made temporary crown to mimic (as closely as possible) the final restoration. As the tissue heals, the adjacent teeth whiten, and the patient evaluates their future smile, the lab is hard at work fabricating the most lifelike crown possible!
