Career ending injury in NBA : Achilles tendon rupture Is it preventable ? What is the cause ? Why ? What can we do? Let's imagine Kevin Durant walked into my o...ffice as a patient. First thing I will ask about the details of present injury at the Achilles but also the history of the previous injuries involved especially at the spine, pelvis, knees and ankles. https://www.foxsports.com/nba/kevin-durant-player-injuries From 2017 to 2019, within these two years he had experienced around 9-10 sprain and strain injuries in his lower leg and the recent one is the Achilles tendon rupture. This is not surprising because ankle injury is the most common basketball-related injury and second is knee injury because of this particular sport required high degree of vertical movements, in each game it required 35-46 jumping and landing activities per game not to mention practices before the game, it also required a high demand of lateral movement and shuffling, a combination of both single and two-footed jumps and landings. These contact mechanisms make ankle injury as the most common injury is basketball players. (Madden et al, 2018) Now let's look at the clinical anatomy of an ankle joint in the picture below, The Achilles tendon is a very thick tendon that connects from your insertion-heel bone to Gastrocnemius (Calf muscle) and it's origin to the femur and soleus to the tibia. When you are landing, the muscles will be stretched and when you are trying to jump, the muscles will be contracted. That's why when Kevin Durant was trying to make a cross over, at that split second of the Achilles tendon from stretched to contracted to provide that push-off movement to the ankle, it torn. Here is the million dollar question: Why ? While I was working with Dr. Ruch and doing researches regarding on ankle and knee injuries. He emphasised on the Joint Neuromuscular guarding reflex, this is not something new or revolutionary to clinical approaches. Remember the last time you rolled your ankle, at the first 1-2 days your ankle is swollen, muscle are all tight and you are not able to move it and bare weight. That's the body provide neuromuscular and cellular response to protect the injured joint to make it stable and heal. Couple days and weeks later the swollen is gone and you gained back some movements and the muscle will still be tight because now the injured joint NEVER got corrected back to it's alignment, that's why the muscle is still tight, if you keep repeatedly stretch it ,ice it, massage it, you may potentially make it WORSE ! The only way is to adjust the joint until the muscle guarding reflex and joint alignment back to normal and stabilize with taping and re-hab with proprioceptive training to rewire the brain <-> ankle connection. Now back to Kevin Durant's Achilles rupture, it is very likely that he had a chronic unstable and subluxated heel bone and knee resulting to sustained hypertonicity and decrease of elasticity of calf muscles.
