Make Your Appointment for a Well Check Up! Summer is the perfect time to do your annual well check up if you have not done one already. Many children require a current well check up for completion of school registration and/or sports forms. If you or your child requires a well check up, please contact our office to schedule an appointment. If you or your child has already completed your annual well check up, you may drop off the school forms for us to fill out.
