Abstract INTRODUCTION: Despite the increasing reports on mechanical aspects of contracted endodontic access cavities (CECs), we believe that the biological aspects (debridement) have not been adequately investigated. This study examined if 1 type of CEC (orifice-directed dentin conservation [DDC] access) was able to debride the pulp chamber, root canals, and isthmus of mesial roots of mandibular molars similar to a traditional endodontic access cavity (TEC). METHODS: Mandibular molars (N = 32) were selected and divided randomly into 2 experimental groups (n = 12) after micro-computed tomographic scanning (group 1: TEC and group 2: DDC) and histologic controls (n = 8). After instrumentation to a size 30/0.06 taper using 3% sodium hypochlorite as irrigant, specimens were processed for histologic evaluation, and the remaining pulp tissue (RPT) was measured from the pulp chamber, root canal, and isthmus at all root thirds. Data were analyzed using 1-way analysis of variance, Kruskal-Wallis, and appropriate post hoc tests (P = .05). RESULTS: The RPT in the pulp chamber was significantly higher in DDC compared with TEC (P < .05). Comparing the root thirds in each group, there was no significant difference in the RPT within the root canals or the isthmus (P > .05). The RPT within the root canals and isthmus was not significantly different between the 2 access cavity designs at any root third (P > .05). CONCLUSIONS: Debridement of the pulp chamber was significantly compromised in DDC. The type of access cavity did not influence the amount of RPT in the root canals and isthmus. It is with great pleasure we welcome Dr. Tes Milan to our practice! Dr. Milan will be joining our practice in August and we are very excited for her arrival. Dr. Marites Tes Milan was born and raised in California. With a father and mother who are both nurses, Dr. Milan was raised with an appreciation for both hard work and changing lives through medicine. Dr. Milan received her dental degree from the University of Southern California and was oered a position with the Un...ited States Army in the Health Professions Scholarship Program. As a general dentist in the United States Army, she was deployed to Iraq providing dental support in a combat zone. It was her experience in the military, treating dental trauma and performing root canal treatment, that she realized that her true passion in dentistry was endodontics. She then received her Endodontic Certicate and Masters of Science in Dentistry from Loma Linda University. Dr. Milan’s goal is to provide high quality patient care and comfort for every patient. She is able to provide several methods for anxiety relief and is certied in oral conscious sedation. Dr. Milan regularly attends endodontic meetings and continuing education courses to stay on the cutting edge of endodontic care. Dr. Milan achieved the highest distinction possible in endodontics when she became Board Certied in Endodontics and is currently a Diplomate of the American Board of Endodontics. Dr. Milan has practiced for several years in the Bay Area and has relocated with her husband and 3 children to Southern California to be closer to family and friends. She is excited to work with her former co-resident Dr. Justice and is eager to meet and provide endodontic service to this wonderful community. Dr. Milan is currently a member of the American Association of Endodontists, American Dental Association, California Dental Association, and the Omicron Kappa Upsilon Dental Honor Society.

Warattama Suksaphar, DDS, Danuchit Banomyong, DDS, PhD, Titalee Jirathanyanatt, DDS, Yaowaluk Ngoenwiwatkul, DDS, PhD Survival Rates from Fracture of Endodontically Treated Premolars Restored with Full-coverage Crowns or Direct Resin Composite Restorations: A Retrospective Study Abstract Introduction The aim of the present study was to compare the survival rates against fracture of premolar endodontically treated teeth (ETT) restored with resin composite or crowns and to identify risk factors associated with the fracture. Methods Data from dental records and radiographs of premolar ETT with postendodontic restorations (ie, resin composite or crowns) were collected between 2012 and 2016 and selected following selected inclusion and exclusion criteria. Tooth location, type of restoration, number of proximal contacts, and amount of tooth surface loss were recorded. The incidence and restorability of postendodontic fractures were identified. Survival rates against fracture of the 2 restoration types were calculated using Kaplan-Meier survival analysis. Any potential factors associated with fractures were identified using Cox proportional hazards models. Results The survival rate against fracture of ETT restored with crowns (95.1%) was higher than resin composite (77.0%). ETT restored with resin composite with 1 or 2 tooth surface losses and 2 proximal contacts had a high survival rate of 88.5% that was not significantly different from ETT with crowns. A higher incidence of restorability after fracture was observed in teeth restored with resin composite than crowns. The type of restoration and number of proximal contacts were identified as potential risk factors associated with fracture incidence. Conclusions The survival rate against fracture of ETT restored with crowns was higher than resin composite. However, ETT with 1 or 2 tooth surface losses and 2 proximal contacts and restored with resin composite showed a high survival rate that was comparable with ETT restored with crowns.
