#extraction #saturday. Gotta love some cow horn action to start your Saturday morning. Severely infected molar that was deemed non-retreatable by an #endodontist. Notice some external resorption of the mesial root due to the chronic infection that was present. Site was cleaned and grafted using CGF/PRF protocols for a future implant. You want to make sure you throughly clean out these infections prior to grafting even if infection is around the sinus. Small perforations can ...be dealt with using CGF/PRF membranes to help seal perforations as well as other collagen membranes/tape prior to grafting along with antibiotic treatment if needed. How do you maintain the gingival architecture and volume with your patients when it comes to anterior implants? These custom healing abutments are one way I have enjoyed maintaining my patients existing papilla, volume, and contours while at the same time minimizing risk that can come with an immediate temporary implant crown. By all means a temporary crown is great for the patient when able but if that is not feasible or desired this is the best alternative in my opinion. This tooth had some beefy roots but nothing these cow horn type of forceps couldn’t handle...This extraction reminds me of a movie called Rat Race where one of the kids said they were prairie dogging it. Love the shot of the internal of this implantDo you place your bone level implants supracrestal, level with the crest of the bone, or subcrestal? Keep reading and I’ll give you my preference and why. This case is the follow up from my previous video of placing the 5.0 diameter C1 implant from MIS @misimplantsusa. This patient presented with a failing bridge that needed to be replaced so instead of doing another FPD (fixed partial denture) we decided to have a more natural o...utcome and one that was easier to clean with individualized teeth. We sectioned the existing zirconia bridge and began the implant process and the implant at the time of placement had excellent initial stability. When placing bone level implants my typical preference is to place the implant 0.5 to 1mm subcrestal. There are many reasons for this decision and here are a few: improved tissue thickness, improved long term crestal bone levels (especially in patients that don’t heal as ideally), irregularities of crestal bone levels/defects, and improved outcomes in patients that may be more prone to periodontal disease. In some cases that can be more compromised due to a variety of reasons I may place the implant even more subcrestal than 1mm. Swipe to see all the photos of the surgical process. I often try and evaluate different products and systems to better treat my patients in various situations. I’m really loving the simplicity of the MIS C1 implant system @misimplantsusa. It's a conical connection implant that comes with the final drill and has a really simple workflow. In reality, this is often what we see in practice when it comes to full arch cases in many patients. These patients have very little quality keratinized tissue. These are great cases to help improve tissue quality and quantity, however, many patients do not want to do extra procedures due to cost, time, additional healing, etc. So what do you do? It is imperative to have a clean calculated incision when uncovering these implants. Your midcrestal full thickness incision needs ...to carefully split what little keratinized tissue you have in order to have your abutments/locators surrounded with the best tissue possible. That’s what we were able to do with these two cases shown in preparation for lower overdentures. You may ask yourself what is so special or difficult with this smile? Let me tell you... This patient presented with a failed root canal and tooth on the upper right central incisor that needed to be extracted (swipe to the second photo to see the before photo). This patient had very high expectations and wanted a single implant but no other treatment or crowns/veneers done. As you can see this patient has a high smile line, diastemas on both sides, and the majority of her sm...ile focuses around the pink zone (tissue) and the white zone (teeth). With this information it was imperative to maintain this patients gingival architecture with little or no resorption to maximize the final esthetic outcome. We were fortunate enough to place an immediate implant but didn’t want to place an immediate implant crown due to the patients bite, however, we needed to preserve the patients papilla and tissue so we performed the next best thing and placed a custom healing abutment after performing a dual zone bone grafting technique.
