Before this patient received multiple treatments with a Q-switch laser from another clinic, she had one problem - dark patches. After her treatments, she said she picked up another problem - white patches. She became depressed and wouldn't get out of the house. Her sweet supportive husband found me I said that Q-switched lasers is one way to improve pigmentation and doctors use this technology as part of the standard treatments nowadays. But like other treatments, it's not ...perfect. I said I'll try my very best to BLEND her unwanted patches and prevent them from getting darker but I also told her the truth that her particular type of dark patches may never go away and there's NO PERMANENT treatment for it. Sometimes, truth hurts but wins in the long run. By knowing the truth, hopefully she'll avoid snake oil fixes that include useless expensive creams. #acnescars 101. This is a typical patient who sees me...who have done lots of treatments somewhere and allegedly no results. This patient has Grade 4 #scars that consist mainly of deep #icepicks exacerbated by strongly tethered skin to the underlying tissue. The tethered skin is represented by the dent like as if a meteorire blasted it and created the large crater. It's called rolling scar. Actually, the underside of this crater consists of thick, hard, tumbleweed-like scar...s firmly anchored to the subcutaneous layer. You cannot simply inject fillers or nanofat here because this area is tight. The scars need to be loosened and release d through a technique called #subcision. Only after subcision will propping it up with #fillers or #nanofat makes sense. The icepicks are much, much more laborious to treat. I would take ten tethered rolling scars versus one icepick. These scars will laugh at #lasers, #prpmicroneedling and #rfmicroneedling. Yesterday, this patient had subcision followed by #radiesse injection followed by 3mm plain #microneedling followed by #nappage technique #PRP. The goal of the 1st phase of the treatments is to lift the skin and collapse the icepicks. When the icepicks are more level, then lasers make sense as icing on the cake. Realistic timeline to improve up to 80%: 3 years. Frequency of treatments: every 3 months. Lots of patients ask me, "Is there a bright side to my scars?" I always say, Yes of course! See more
