This client has to use her neck to create space in her thorax to breath in. She is trying to elevate her ribs using neck muscles to put oxygen in her lungs. I was amazed about this when I dissected with Thomas Myers (Anatomy Trains). This shows a continual tissue connection of our body from bottom to top. Not addressing big guys here such as diaphragm (moving up and down ~20,000 times/day) or tongue position can be the reason why massaging the foot is not getting you better. [Stop tucking your chin] this client was tucking chin and trying to straighten her spine and sit tall all the time. Occipital pain (base of head), neck tightness, migraine constantly. She stopped practicing all the myths of good posture. She learned how to breath to orient ribs in more optimized position for head and neck to rest on. ... Forward neck posture (head is just following the neck, so it’s not really forward head posture) usually have tightness of muscles in your front of neck and they are pulling your neck forward. All these muscles attach to ribs and sternum. So no matter what stretch you do, it won’t be as effective as you work on your rib cage (breathing pattern and positioning of ribs). [Pelvis shifting on legs] These pictures are to show how her pelvis is positioned on her legs in each stance of walking. Right pelvis on right leg looks very different from left pelvis on left leg. This is a classic example of inability to shift weight to left hemipelvis (AF IR on L). ... This is happening every steps she takes. And our brain perceives this as normal. This neuromuscular adaptation make our muscles around pelvis and spine function differently on left side vs right side. What changed? This was taken when his left heel hit the ground during walking. What do you see? Here are some that I see. BEFORE:... 1. Pelvis/low back: When he steps and hits the ground, it looks like he is extending his back. His left pelvis stays in a state of anterior tilt. In AFTER, he has left AF IR if you are PRI minded. 2. Right arm: His right arm goes out to the side. (right arm is reaching forward in AFTER) 3. Wide stance: Because he didn’t have adequate control of frontal and transverse planes. 4. Early left heel strike: Look at right foot in both pictures. Right heel is up all the way when he puts his left heel on the ground in AFTER picture vs Right heel is barely up in BEFORE picture becuase left leg hit the ground wayyyy too early. Patient with right shoulder pain (diagnosis #impingementsyndrome). Often shoulder patient comes in with neck stiffness/pain. I did not show her the pictures I took, she never knew her neck was tilted. 4 visits in 1 month. When Zone of Apposition was achieved, rib cage position changed, scapula position which sits on ribs changed, tension in the neck muscles which attach to ribs changed, then her posture changed. Her pain changed. In the past, the patient tried massage, shou...lder stretch (that famous posterior capsule stretch), scapula exercises such as rowing and lower trap stuff. How about Iliacus? Iliacus is such an undervalued muscle. I see everyone loves working on their glutes for knee pain, back pain etc. Everyone loves to externally rotate femur during squat ignoring this powerful one called Iliacus!!!!! As you can see how the muscle fiber runs, it works more in frontal plane rather than just one of hip flexors.
