Peninsula Healthcare Connection 12.11.2020

our small clinic in Palo Alto at the Opportunity Center, which focuses on bringing world-class care to those that are experiencing homelessness or are at-risk, has received the designation of becoming a Federally Qualified Health Center Look-a-Like (FQHC-LAL) status as of yesterday. This designation enables us to be more self-sustaining in order to help more people. Thank you to our board, and leaders @Eileen Richardson CEO & @Elfreda Strydom COO

We appreciate Santa Clara Family Health Plan’s investment in Peninsula Healthcare Connection as we lay the groundwork together for the Health Homes program. By providing a medical home for the homeless we can address the full range of a member’s physical health, behavioral health, and long-term services and supports needs!
