Concepts of alar cartilage preservation have been part of my practice for over a decade, having published one of the first papers on this concept in 2009. A newer way of thinking of this is ‘bidirectional tip control’, as shown in this animation. Of course the concept is more complex than this, but this video shows briefly how one can control tip projection and columellar show with the tongue-in-groove method. Adjustments to the tip and tip-lobule position and length can then be made with an alar-spanning suture. No cartilage is removed from the tip. Frequently, we can avoid dome sutures as well. Mastering the technical aspects of rhinoplasty is only one part of the equation, as experienced rhinoplasty surgeons know. The psychology of patients, understanding their expectations and counseling them properly preoperatively are also big challenges. Our research group has sought to understand more about these issues, in order to help patients and surgeons alike. This recent paper is (to our knowledge) the first to prospectively measure expectations for outcome, both fun...ctionally and aesthetically, in a cohort of rhinoplasty patients. We see that while aesthetic patients express great satisfaction with their results, there is a tendency to ‘want just a little bit more’, compared with functional outcomes. The impact of social media on these expectations cannot be ignored. It’s been quite a journey. From falling severely ill as one of the first COVID patients in the Bay Area in late February to being allowed to get the vaccine with the first group at Stanford. I know there are nurses and staff taking care of COVID patients who are also getting this nowI’m comfortable being in this group because my immunity has likely waned and I’m at high risk for exposure again. I hope this is the first step to normalcy for all of us....
