About Making a Difference Beyond the Practice After a few years in practice, we as doctors hit our stride. The steepest part of our learning curve slows. Hospital rounds become routine. The 1,000th outpatient consult for chest pain is no different than the first. And then a 7 year old girl from the Philippines gets admitted to the floor with unoperated Tetralogy of Fallot. She's more cyanotic than any kid you've ever seen. Her O2 sat is 72%. Her fingers and toes are profoundl...y clubbed. Her hematocrit is 65%. Sadly, her family has known about her diagnosis since she was born but they could not afford to come to the U.S. And now here she is, right out of a textbook. It is like Helen Taussig is describing one of her patients directly to you. The excitement of the unique and bizarre cases from our training rushes back. But these fascinomas come far and few between. Inspiration from 8,000 Miles Away Enter Baby My, a precious 2 month old girl with a newly diagnosed double inlet left ventricle, transposition, and hypoplastic aortic valve and arch. A single ventricle in need of 3 stage palliation seemed the right direction to head. We had to consider the possibilities of pulmonary hypertension and sift through the reported diagnosis of pneumonia, none of which seemed too difficult to tease out. That is, if she were in the United States. But she was 8,000 miles away...in Vietnam. Although the obstacles were many, we embarked on this journey to provide the highest quality medical care and decision making, all by e-mail. A Telemedicine Platform Created For Doctors, By Doctors At the end of the tumultuous but incredible 3 month journey, Vietnam had completed its first Norwood and Baby My was its first survivor! The countless e-mails and hours spent on the case caused us to consider whether a more refined system of communication was available to care for such patients. This forum could help, provide care for, and save vast numbers of patients around the world. With this in mind, MD1World was founded. We built MD1 to serve as a unique forum, an online platform of telemedicine, that allows and encourages physicians at the forefront of medicine to engage and support physicians and patients both locally and worldwide. We believe you will impact even more lives through your involvement with the MD1World community, as you journey with fellow colleagues and patients to lend your expertise across the nation and the world.
