I see this quite often. Bathroom exhaust fans terminating in an attic. Warm, moist air in a cold attic can cause condensation. Notice the discoloration on the OSB. Mold?, Mildew? Rot? Just staining? Hard to say, but OSB can and will fail with sufficient moisture.
