While reviewing comments under a General Aviation incident report (KathrynsReport), a commenter noted that a similar takeoff accident had happened to a CFI flying with a student. In that accident, while rotating off the runway, the CFI's seat slipped back and she instinctively grabbed the yoke. An accidental stall caused by the seat's movement ended up costing the CFI her life, serious injuries to the student and the loss of an aircraft. So this morning, I went looking for c...hecklists to see how many of them include, "Check your seat position, be sure it is locked in." I didn't find one and for some aircraft, the seat rails have a long history. What I did come across are some startlingly bad numbers on how many takeoffs are preceded by a complete and thorough Pre-Flight check. Here is what Wikipedia said... "Following a checklist would have shown that the gust lock was engaged on the Gulfstream IV crash on May 31, 2014. The National Transportation Safety Board downloaded data from the aircraft's recorder and found it was a habit: 98% of the previous 175 takeoffs were made with incomplete flight-control checks. The National Business Aviation Association analyzed 143,756 flights in 2013-2015 by 379 business aircraft and only partial flight-control checks were done before 15.6% of the takeoffs and no checks at all on 2.03% of the flights." At least 17% of business aircraft flights had incomplete preflight checks or none at all. You don't have to search too far to find reports on incidents that stem from a seat moving. To be fair some of the seat movements are due to worn seat rails, but there has been an issue with seat-latch design. A lawsuit after one incident forced Cessna to reveal that it had know there were problems with their seat-latch design, but until 1996, had only installed the improved version on higher end aircraft. Also, Cessna noted that it had not retrofitted 125,000 aircraft that had the poorly designed seat latches. It is likely everyone has suffered a seat sliding in their car and many have had the same thing happen in a plane. In either situation, the result could be unfortunate, but at least in your car, when you grab the steering wheel, you won't pull up into a stall. So even if you are the only person that flies your plane, double check to be sure your seat is securely locked into position. About preflight check lists? An analysis of a USAF T-6A Texan II lost in an inverted stall. Incident was 1 May 2019 and the post was 15 Feb 2020. Post includes both a summary of the pilots actions and the final report of the USAF.
