I have worked non-stop, in-person, 12 to 15 hour days for the past year. No days off. The pandemic was back-breaking in so many ways. I never complained to my... office staff, or to my patients, or to my family or friends. I take my own calls, so I'm never off and I answer every single email sent to me by patients immediately, and they all have my email. I was never away from my phone. I just kept going. But, I have felt overwhelmed and burnt out so many times. I just kept praying. I felt blessed to be in a position to help so many when the world was falling apart. I treated hundreds of people I've never even met. My office has been open and ready to serve, in person, not just on zoom like so many others did. More than half of my patients live at poverty or below poverty and I treated them all to my fullest. Fewer than a handful of my patients were hospitalized. Today is the first work-day I took off in a year. To recharge. To relax. To unwind. All that matters is that looking back my conscience is clear that I showed up and gave it all I had... and I'm hard on myself. Those of us who made it through the pandemic should be grateful that we are not just a statistic. We are the lucky ones. So many died and so many will suffer for a lifetime. We should never take our health for granted. We should always talk more about our blessings than our problems. I was once suicidal. I get it. Last night, one of our ER docs committed suicide. In the past decade, I've personally known four doctors who have killed thems...elves. One shot himself and his family. One jumped off of the hospital roof. One hung himself. Another intentionally overdosed on prescription drugs. In America, 400 + doctors a year kill themselves. More than any other profession, by far. One in four medical student has significant depression. One in ten has grappled with suicidal ideation. When I got into med school, I felt like an imposter. I felt like the dumbest person in the class. There must have been a mistake. How could they let someone like me in? The schooling is rigorous and dehumanizing. You are competing with those who seem perfect and you are so imperfect. You are constantly broken. Med school is an emotional boot camp. Emotional abuse and physical abuse is rampant by senior doctors who themselves were abused. Many pride themselves on making students cry. Repeatedly. Oh, and the lack of sleep. And dealing with disease and death. Giving the news of cancer to patients. Operating on and giving treatments to patients who are going to die no matter what you do anyway. And all this happens in your twentys. You have no life outside of medicine. I battled clinical depression for a couple of years. Most of us cried every night but didn't tell each other. I had regular thoughts of suicide. I was ashamed to tell anyone. It breaks my heart when I see physicians kill themselves. This is NOT a selfish act. Suicide is end-stage depression. Just like death is the final stage of metastatic cancer, suicide is metastatic depression. The reason people kill themselves is that they feel totally disconnected from their profession, their family, their friends, their community. They feel alone. I was there. These feelings are wrong. Your brain tricks you. The feelings will pass. They always do, if you wait. You are not alone. You are loved. You are valued. You are God's gift to this world. Please, if you have thoughts of suicide, call the suicide hotline. Go to the ER. Call a friend or a loved one and just hug and hold them. I'm also here for you if I can talk to you. Suicide is never the answer. Your loneliness is temporary. There are so many ways we can help you. Most importantly, so many of us need and love you. Life is hard. God damn hard. But, there is light and the end of your dark tunnel. And you are not alone. Many people feel the way you do. I was there. It will get better. So much better. I promise. See more

I was just fired by one of my patients because a (sorry to say) naive physician spoke poorly of me. She is a 60 year old woman who called my office for not fee...ling well. I got her in right away and her exam was normal. She had a urinary infection which explained her symptoms. On a hunch, I also ran a blood test to check her cardiac enzymes (which most doctors don't do) and this came back borderline abnormal. Given her lack of cardiac symptoms, a normal ECG, a normal Echocardiogram and a clear urinary infection and two decades of experience, I decided to treat her with antibiotics which made her feel better, and send her for a cardiac PET scan to evaluate any blockage in her heart. The scan showed a large area of blockage with no heart attack. It took me over an hour in the middle of my busy day to get her admitted to the best team and have her heart stented and sent home the same day. But, one of the members of the team, told her and her family that I mismanaged her by not admitting her the minute the enzyme was abnormal. Unless there is gross negligence, I always build up other doctors and give them the benefit of the doubt. To speak poorly of other doctors without knowing the entire picture, playing "hero", or Monday morning quarterback is silly, immature, and frankly professional jealousy. My practice is busy, and I don't need patients, but because I truly care about patients as human beings and not as a paying customers, this really hurts. Whatever profession you're in, don't speak poorly of others to make yourself look better. It's short sighted and as I always say, nobody ever got stronger by putting others down. I will take comfort in the fact that God knows the truth, that I saved her life. In the end, it's between me and my conscience and my Maker. Next. See more

I am so proud of my practice. Almost ALL my patients over 60 have been vaccinated. Now, I'm fighting to get those over 30 with risk factors vaccinated. I've ...been working non-stop since March of last year. No breaks- many days over 15 hours a day, often totally overwhelmed with patients in the office, calls, telemedicine. I've only had a couple of deaths in nursing homes but treated all patients in time and almost everyone out of the hospital. It's been a rough year, but memorable. We helped hundreds of people who were not our established patients recover. What a meaningful journey. It's in difficult times that you learn true character of people. I am grateful to my office staff for all their hard work, while many physicians ONLY did telemedicine, our office was open the entire past year. See more

I've been accused of flip-flopping on the pandemic. Let's discuss it. Because, most people confuse science with politics. If I ask you your position on capit...al punishment or same-sex marriage or abortion, you would be flip-flopping if you changed your mind every few weeks. These are topics which you can research and take a position on. Our knowledge changes little over time. Perhaps you become more conservative as you get older. Perhaps every decade there is some scientific evidence that affects your thinking. But, the change may happen over decades. This is not true of a viral pandemic. A novel virus is identified. No one knows how it will act. Who is at high risk. Who is untouched by the virus. Are there risk factors? Are there treatments? What is the morbidity and mortality? Will it burn out on its own? Models try to predict the future based on the past. They often fail miserably. We are in uncharted territory. Science REQUIRES that you change your mind when new evidence is presented. If you stick to one belief, because you want to be right, or because it serves your ego, or because you filter your opinions based on your political biases, then that's not science. I wear two hats at all times. What does Judaism ask of me? What does science/medicine ask of me? Both place human life at the highest ranking. Judaism places hope, non-despair, as essential. With a new virus, you lockdown, to flatten the curve and to identify the most vulnerable. But, you CAN'T stay in that position indefinitely, because life is more complicated: we are always confronted with more than ONE variable. At some point, depression, anxiety, suicide, domestic abuse, obesity, alcoholism, smoking, medication noncompliance end up killing more people than the deaths from the virus. Science demands a continuous analysis of the multi-variables. At some point, you become like the race-car driver speeding through unknown terrain. If you stick to always turning right at the next curve, you will crash and burn. You must take in new data and remain flexible to change directions with the end goal being doing good, saving lives, not taking away hope, not killing people with another disease while trying to save them from the first. It requires learning. It requires admitting you don't know everything. It requires humility to realize when you were wrong so you can change directions, even back and forth. This is NOT flip-flopping. This is science and medicine and virology. That is why I block people who show up with nasty comments constantly to criticize, all the while when they themselves never express their opinions or think outloud, because those who do will at times be proven wrong. I realize most of you understand this. But, it needed to be said, in our divided and highly politically charged world. My intentions with all of my posts are always to help. If I know something I share it. Not to boost my ego, but because that's who I am. By nature, I like to share and help. I also trust that people whose intentions are pure and who follow me know that I am doing my best to help but that I am not the ultimate authority on anything. These are merely my opinions.
