Healing Arts Physical Therapy 12.04.2021

Last week a student of ours from Sacramento State University successfully used a treatment called NMES which stands for Neuromuscular Re-Education. NMES sends electrical impulses into your muscle to get it to contract properly. He used this on a patient who's quadriceps muscle was not contracting properly after a knee surgery. The quadriceps muscle is responsible for extending the knee and stabilizing the knee during activity such as walking. This patient before the treatm...ent was still reliant on a single crutch to walk and did not fully feel confident in her knee to support her weight. We performed one session of NMES on her and low and behold the next time she attended therapy she had ditched that crutch and was walking much more normally! We will be using this type of treatment much more often here! I have been hearing about so many elderly people falling these days. Falls which result in broken shoulders and hips, of which can be disastrous and deadly to people in their older years. The reason for this rise in falls is due to people staying at home more due to COVID-19 and not getting the necessary exercise and fitness one needs in a day. People are staying home more, staying in bed longer or sitting on the couch longer to read a book or watch TV. Our muscle and n...ervous system need to be exercised and challenged on a daily basis. Unfortunately, some people can get to such a poor conditioned state where it can be dangerous for them to walk and exercise, as one might sustain a fall in attempting to do so. So, they need a service like balance therapy to challenge these systems under a more controlled/safe environment. We have some very helpful equipment here that can significantly improve a person's strength and balance to prevent falling and injuring one's self. One device that I am especially partial to is the Solo-Step; pictured in this post. Ones walking, balance and strength can be challenged and improved using this device and one will have the assurance and confidence that they will not fall during the therapy. When they are attached to the harness and lanyard along the ceiling rail system, they can be sure they won't fall and will be able to push themselves further than normal. This fortunately excites the nervous system and allows it to adapt and learn and improve the muscles coordination, performance and strength. One needs to be able to be able to walk and move their head, change directions, and walk backwards in order to be safe. All these components can be challenged and refined with our balance therapy program. If you know of someone whether it be a family member or friend that is struggling with walking and balance and may sustain a fall, then tell them about this service we offer.

Let me introduce you to a muscle called the posterior tibialis. It's our arch lifter; the muscle that lifts up the arch of our foot. In many cases where our arch is collapsing (a condition called hyper pronation) this muscle is specifically weak and not doing its job. When the arch collapses when standing and walking it can lead to stress to another structure that supports that arch called the plantar fascia and thus plantar fasciitis can occur. It’s like when one perso...n on a team doesn’t carry the weight, the other members of the team feel the stress. Sometimes the tibialis posterior itself can be painful at the tendon on the bottom of the foot or at its attachment along the inside of the leg. The specific condition here is called shin splints and another condition that is just plain not fun to have! I have also seen patients that have torn the posterior tibial muscle which can lead to a severe collapse of the arch and be very painful and may even require surgery. Being sedentary, overweight and deconditioned can lead to problems in this muscle. So, addressing these conditions will help. Included in this post is an exercise we commonly give people to gain motor control and strength back to this muscle. It’s called an arch raise exercise. It seems simple, but if you really focus on doing it right and do it many times you can really feel the muscle getting a workout! ARCH LIFTS Start with your foot on the floor. Raise up the arch of your foot while maintaining your big toe, ball of your foot and heel on the floor the entire time. Try and do this 30-50 times and do 2-3 sets. We offer telehealth services at Healing Arts Physical Therapy during the pandemic. On occasion we have patients cancel their appointments because either "they are not feeling their best" or they have been exposed to a person with COVID-19 and are unable to attend appointments. Our Physical Therapist's will be able to spend 30 minutes with you through a virtual visit and help guide you through exercises and movements to address mobility problems and pain. If you are struggling with staying active due to pain, weakness or other medical condition this could really help you! This is not the time to be sedentary as activity and exercise helps to improve the immune system and helps with depression and anxiety.
