Q: I am a practitioner of 20 years, and I am seeing a trend evident among many practitioners in my area that is disturbing to me. It has always been my belief that, as dentists, one of our most significant professional responsibilities is to assist the public on how to keep their natural dentition for as long as possiblepotentially for their entire lives. Yet, some practitioners are eager to accomplish root canals when teeth look even slightly questionable and remove teeth t...hat could be easily restored. Amalgams that could serve for years are being removed only for esthetic reasons, taking away viable tooth structure and further weakening teeth. I am also seeing wanton removal of restorable teeth and unnecessary removal of bone to facilitate the placement of dental implants. Other colleagues have also commented to me on this current state of the profession. You are well known as a reputable dental speaker. What is causing these observable changes in the profession, and what can be done about our apparent shift in emphasis? A: Unfortunately, your stated observations are correct. I am greatly concerned about the conditions you have described and many others. In my answer to you, I will state my own observations about the current state of the profession and how in our own practices we can help our patients better preserve their natural dentition for life. There is an obvious movement toward measuring practice success by dollar production. This has happened because of numerous factors to move the profession toward business characteristics and away from professionalism. Using root canal treatment only when needed Properly accomplished root canal treatment is a necessary technique, but I am seeing some practitioners move too rapidly to devitalize teeth and perform root canals. Current pulp capping proceduresindirect (no pulp exposure) and direct (pulp exposure)should be used more often, reducing the need for root canal treatment. The following characteristics should be present for successful pulp capping (figure 4): Healthy patient, preferably with apparent tooth vitality present according to testing No pulp exposure or a small (0.5 mm to 0.75 mm) exposure If pulp exposure, there is slight red blood exudingnot magenta, not serous, not purulent Use of popular, proven pulp capping materialsexamples are Vitrebond Plus (3M), Fuji Lining LC (GC America), MTA pastes from Ultradent, Dentsply, Avalon, and othersaccompanied by informed consent for the patient about expectations of the indirect or direct pulp cap https://endodonticsla.com/saving-teeth-the-rush-to-preserv/ #rootcanal #endodontist

What was the toxin causing my painful symptoms, and where was it coming from? After further research, I suspected that I had infections in my root canal teeth. Infections trapped in the teeth give off all sorts of toxins, and there are many documented cases of people getting better from trigeminal neuralgia and other medical conditions after removing their root canal teeth and the attached periodontal ligament. I had four such teeth, and I was very encouraged, as it appeared ...that I had found the source of the toxins. I contacted Dr. Stuart Nunnally, a dentist in Marble Falls, Texas. He agreed that it was likely that I was suffering from infections, and he requested a panoramic X-ray. I had the X-ray in Ohio and sent it to him digitally. He also suspected that my wisdom teeth extraction sites were infected, believing that I had at least four sites of infection, perhaps even more. He did a cone CT scan at his office and found that I had nine infection sites: four root canal teeth, one crowned tooth, and all four previous wisdom teeth extraction sites. When the pathology report came back after surgery, I had more than 15 serious infections! I received intravenous vitamin C, as well as platelet-rich fibrin plugs (made from my blood), which studies show are very important for a speedy recovery. I never even took an antibiotic. It was very difficult for me to get to Texas for the surgery as I was too sick to fly. My sister flew into town to drive me and my mom, and we had to pack all my food, with me lying down the entire trip. It took us 3 days, and we had to drive through storms and flooding. Within a week, I was starting to cook for myself, and I noticed an improvement in my voice immediately. It continued to improve steadily for weeks until it was completely normal all of the time. There are plenty of studies linking all sorts of medical conditions to root canal teeth, but dentists still carry out millions of root canal procedures each year. I encourage anyone who suspects that they have a health problem because of their teeth to see a biological dentist to determine whether treatment may be beneficial. If you ever experience pain and or swelling in your mouth- especially after seeing a dentist and had a root canal done, talk to your endodontist immediately. What is a Rubber Dam? Simply what a rubber dam does is isolate the tooth or teeth to be treated as well as provide a barrier from bodily fluids i.e. blood and saliva. It covers you the patient’s mouth with holes punched in it for the teeth or tooth to be treated. The advantages are immense. The dentist can clearly see the area of operation with no distraction such as tongue or cheek movement. ... You the patient can easily swallow normal flow of saliva with no fear of swallowing any of the materials used by the dentist such as filling materials, dental cements or even dental instruments that might fracture during use. Almost all dental materials are adversely affected by moisture when they are being placed. If this contamination of moisture (saliva, water, blood) occurs during placement it will adversely affect the outcome of the procedure in almost every instance. Moisture control and isolation of the area to be treated are primary to the success of any dental procedure. A Tennessee dentist didn't sterilize equipment, and now his patients need HIV tests. Selecting an experience and ethics-bound dentist and endodontist is absolutely wise. In todays highly competitive and deceptive business environment, the temptation to cut corners and inflate profits is overly tempting. According to a July state health professions board disciplinary action report, Nabers allowed dental assistants to fill cavities and place permanent crowns while he worked as ...the sole dentist at his two offices. His staff also did not properly sterilize equipment, and Nabers submitted forged certifications. The state put Nabers' license on probation for two years and fined him $11,000. Tennessee health officials also sanctioned dental assistant Tammy D. Crye after learning she performed procedures she wasn't authorized to do. She admitted to performing 15 dental procedures outside the scope of her dental assistant licensing between December 2016 and September 2018, according to Tennessee Department of Health records requested by Knox News. Her license is on probation for three years beginning in April 2019, meaning she's still allowed to work. Nabers' fine is the third-highest civil penalty the state has levied on a dentist since January 2018, according to the disciplinary reports. Along with these penalties and a long list of requirements before he can be reinstated, Nabers was found responsible for up to $5,000 in investigation costs.
