Wise words from a fellow PT (and one of my former SportsMed students). Avoid back surgery at all costs and use it only as a last resort. Exhaust all conservative means first. It seems his back pain persists, and now experiences chronic migraines due to spinal fluid leakage. If the head coach for the Golden State Warriors, who likely undergone surgery by a top notch neurosurgeon, had such poor results, what should the average Joe think about back surgery? Having treated numerous patients before and after spinal surgery over the past 5 years, here are some things to consider when it comes to back pain and surgery. Surgeries for the spine can be very complicated. Factors that will affect the outcome include: the surgeon, type of procedure, patient's health history, prior level of function, pain level, source of symptoms (Eg. muscular, intradiscal, neurological, bone structure, cystic) just to name a few. Because one back surgery is so different from the next, you should never make a decision for or against surgery based off of the experience of just one friend, family, or celebrity. Generally speaking, back surgeries do have lower success rates compared to surgeries for the extremities (eg. shoulder, knee, hip), mainly due to various aforementioned factors that could cause back pain/dysfunction in the first place. Again, low back pain is complex and varies greatly from one person to the next. It is always a good idea to exhaust conservative approaches, especially #physicaltherapy, before saying yes to back surgery. How experienced is the surgeon? What is the typical success rate? What are common complications with the surgery? What are my other options beside surgery? Do the risks out weigh the benefits? Asking these questions will help you make a better decision. Good luck to Steve Kerr and hope he recovers soon!

People walk around with structural abnormalities their entire life and never experience symptoms of pain. While others get various imaging tests done (Ex. MRI's) that are read as "normal" - yet they show extreme symptoms of pain. For over a year I was the classic example of a pain sufferer. I identified myself with my pain. I obsessed over my shitty scoliosis spine and for a while I saw movement as a threat. I feared getting worse and I feared never feeling normal again. My fear fed back into my pain cycle _______________________ DO NOT OBSESS OVER STRUCTURE. 1 If pain is present - treat the pain (Ex. manual work). 2 Train the non-painful by introducing safe low threshold non-painful movements. 3 Gain mental and physical confidence in those movements. 4 Introduce progressive and higher threshold movements. _______________________ Not one person has the right answer. We are all different and react differently to pain and treatment. Whether it's someone learning to walk again after surgery, a mother with back pain learning how to pick up her child correctly, or in this case an MMA fighter needing to get back in the ring, our goal is to help make those wishes come true. #PhysicalTherapy #BestOfFremont Had to work around a complete MCL tear during this camp,... it was rough and painful...the most painful injury I've ever had but with the help of god, my team @csagym the amazing team at ESPT physical therapy in Fremont I was able to show up as close to 100% as I possibly could've. This is a a fight that almost didn't happen for multiple reasons lots of thanks are in order but I'll focus on two now. We can now share the fact that about 5 weeks ago Gaston suffered a total MCL rupture/tear at wrestling practice and his debut was in jeopardy. Once the MRI's were done and confirmed the next day he said he still wanted to fight, I told we needed to talk to his Orthopedist and begin Physical Therapy. With the guidance of his DR and the best PT in Northern California Jacon at @elitespt it was established that he would not opt for surgery and focus on rehab. I told Gaston I would only let him fight if he got clearance from both his Orthopedic Surgeon and Physical Therapist thus began the primary focus of the last month of this camp (Rehab) I would film pad videos of him only from the waist up so no one could see the knee brace he was in. This kid busted his a** this last month rehabbing for hours every single day. The average rehab time is at minimum 6-8 weeks for an MCL tear and that if it's a Grade 1 or 2 ... Gaston's tear was Grade 4 total rupture ... I honestly didn't think he could do it... but this kid is special I've said it time and time again. 2 weeks ago incredibly he received clearance from both his doctor and therapist so we moved forward with the fight. We salute all those starting off the new year with a commitment to getting fitter and healthier in 2017! You need to keep in mind that you can't just add load to faulty movement patterns though. Long hours sitting at the computer, extended commutes, and poor ergonomics with mobile devices all wreak havoc on our bodies. Anything that you can find at Lowe's or Home Depot , should not be in your body!! A patient came to me with arm pain and numbness/tingling. After previously undergoing cervical fusion at C4-5, doctor was now recommending fusing more levels. WHAT?!? Hold Up! After evaluating the patient, we noticed that his poor posture created a lot of stiffness in the THORACIC spine (just below his neck). If you take that into account with his prior fusion, it's no wonder his lower cerv...ical spine was wrecked! So we got started on loosening up his t-spine and educating him on his posture, and guess what? His symptoms improved! So at his last follow-up he told the surgeon he wanted to push out the surgery. Now instead of rushing into another surgery, He's DELAYING SURGERY! Remember, Physical Therapy should always be FIRST! It's more cost-effective, less painful, less invasive, and deals with the real cause.
