Doctor, My Mouth Opening is Restricted!!_ Part 1__ Dr. Yang's TMD TALK (7) Originally, I'd like to close the TMD series. However, last week I saw a patient. He came to me, and his mouth opening was restricted. He can only open around 2.5 fingers wide vertically since 2-3 weeks ago. So, I decided to talk about this. What happened to my patient was the locking of his jaw. There are two major kinds of jaw lockings: Open locks and close locks. ... During my clinical exam, I can feel the jaw clicking and palpate the condyle at only one side of his TMJ. While opening his mouth, his chin is shifted to the affected side (the side I did not feel the condyle). Base of the clinical symptoms, my diagnosis would be the "close lock". Of course, the definite diagnosis requires MRI. In this video, we would discuss about the close lock of jaw. How did it happen? What's the TMJ look like anatomically? What are clinical symptoms? I grinned out 3 sets of TMD devices! What's Next? _Invisalign or surgery _Dr. Yang's TMD TALK (6) Some of my patients they really grinned through more than 3 sets of TMD devices. What's next? In previous video, I mentioned about 70% of the TMD is myofacial pain, which means, the muscle pain. The good news for this is, if your TMD is causing by malocclusion, uneven bite/occlusal contacts, Invisalign or braces (orthodontic treatment) could help with the situation. ... Usually, while your bite is not even, let's say, open bite, deep bite, shifted jaw, etc, your jaw can not find a stable position to rest, especially, when you are sleeping. Instead of resting, what it jaw does is constant moving. However, due to the malocclusion, there's many interferences during the process of jaw movement. Sometimes, your jaw would need to shift toward certain angle to be able to perform the movement. In the long term, it would cause the tension of the muscles and the TM joints. The other situation is your joints are degenerated. In this way, the temporary relief is injecting Botox or Lidocaine. For long term, surgical innovation could be the solution. There are many different situations of degenerated joints. You might have to work with your surgeon for further accurate diagnosis. This concludes the entire series of TMD. It hurts along the side of my face_ Dr. Yang's TALK about Temporal Mandibular Joint Disorder (TMD)_1 Temporal Mandibular Joint Disorder (TMD) is a common situation happens to one of the most complex joint system in our body, Temporal Mandibular Joint (TMJ). Usually, it associates with chronic facial pain. The TMJs are the joints connecting our jaw and skull. With the harmony of the muscles, ligaments, and bones, it can allow mandible performing many movements, i.e. chewing, ...talking, yawning, laughing, etc. TMD happens while the harmony broke. Most common symptoms are muscle pain and muscle stiffness around the jaw area. Moreover, patient might hear the clicking sounds while opening their mouth exceeding certain range. In a more severe situation, patient's jaw could be locked. There are three major categories of TMD: 1. Myofacial pain (most common): muscle/facia pain 2. Internaldegeneration of the joint(s): dislocated jaw, displaced disc, or the injured condyle 3. My Invisalign Express 5 Minor Front Teeth Correction Result_ Dr. Yang's Invisalign Journey_ Day59(5) Two months fly by. I completed the Invisalign Express 5 treatment. I am pretty impressed with the result. Treatment Period: July 31st, 2020 - September 27th, 2020 (about 2 months)... Aligner Change: 2-week IPR: No Attachments: No My lateral incisors were a bit inward and tooth #9 was a bit outward and tilted toward the left. I have to wear the trays in anyway. It dose not feel like extra work to me. Haven't heard back from dream schools for interviews? What can you do? _Dr. Yang's Dental School Application Series (5) Congratulations of reaching this step! You have been working so hard, putting up all the information together, and submitted your applications. What if, you haven't heard back from the schools you'd like to attend for interviews and it is already in December of the year. Here, Elya suggests us to send out few follow-up emails to the schools you desire. Upda...ting them few things: 1. You are still interested. 2. Why you will be a good fit for the school? 3. Something specifically about the school that you really like. 4. Update things you did since sending the application in July. From Elya's experience: 1. Do not stop your extracurricular, sports, research, or shadowing. In this way, you will have something you can update to the schools. 2. Schools like students that really want them. They might give you a chance of interview over someone else, if you show your sincere interests. 3. All schools responded her emails, and she received phone calls for interviews few days after that. 4. Even you did not receive acceptance after December, you still have chances. How to whiten my teeth at home using Invisalign? _Dr. Yang's Invisalign Journey_ Day 14 (3) While talking about a great smile. Straightening and whitening your teeth are going side-by-side. Many patients asked about teeth whitening after Invisalign treatments. Today, I am going to share a great news. You can perform teeth whitening using your existing Invisalign trays even during the treatment. ... In fact, every Invisalign patients treated at our office will receive a FREE set of 15% Opalescence whitening gel 1-2 months before their last few sets of aligners. You can also use any whitening gel that available in your area. Methods: 1. Identify the last teeth that would show when you have your happiest smile. For many people, it could be the second bicuspids. We only need to apply the gel toward those teeth, not necessarily to apply to the last molars. 2. Apply the gel at the center of the "cheek side" of each teeth inside the tray. The amount of the gel required is around the size of coriander seeds. You do not need a lot. 3. Place the tray back to your mouth and sleep with them overnight. Few Reminders: 1. Always store the whitening gels in the refrigerator. Gel will start degrading ones it is opened or expose to temperature above 82F. 2. There's a limited range for a person's teeth could be whitened. 3. Avoid food with colors. Including coffee, tea, red wines, ketchup, curry, etc. 4. Sensitive teeth. Apply the gel every other day or rinse the gel away after wearing it for an hour before going to sleep. After all the hard works and tedious application processes, you start receiving interview invitations. Here comes this video guide. ... How to prepare for dental school interviews? For many pre-dental students, like Elya, you might not have many big interview experiences before. Now, Elya has been accepted by four dental schools, and is currently a D1 predoctoral student. She is going to share her secrets and tips for dental school interview with us. 1. Each interview is a bit different depending on the school. It is important to research on the school so you will ready. Elya suggests to go to the Student Doctor Network (https://www.studentdoctor.net/profession/dental/) and see what type of the interview the school had. Some interview could be one-on-one with professors or students, and some could be MMI (multiple mini interview) style with many questions and different kind of procedures. 2. Study the past interview questions from the school, and use the technique "repetition". At the Student Doctor Network (https://forums.studentdoctor.net/forums/pre-dental.17/), people would post the past interview questions, around 10 years, from each school. Elya suggests using "repetition" technique by going through all the questions that was asked in the past and preparing your own answers before hand. You would feel much confident answering the questions while you are in the interview. From her experience, there were many similar questions. 3. The most common interview questions. a) Why dentistry? b) Why the school specifically? c) What can you bring to the school? Elya feels, after above questions, the most questions followed would base on the answers you just provide and go deeper into that. 4. Research the school website and looking for the things making them unique or being proud of. Those could be the mission statement, the clinic, the way they teach, or the community outreach. After diving into the website, Elya suggests to see what aspects you would be interested to contribute to and connect back to your past experience as you can. The more specific about the organization the better because they know you really look into them. Will grinding or TMJ problems affect Invisalign treatment?_ Dr. Yang's Invisalign Journey_Day 1 (2) I am going to share my day 1 Invisalign journey after wearing the aligners overnight. Oftentimes, I got to ask these questions "Dr., I grind my teeth a lot. Can I still wear Invisalign?" ... TMD, is the abbreviation of temporomandibular joint disorder. TMJs are the joints holding each side of our jaw and allow us to be able to chew or perform jaw movements freely, i.e. open/close our mouth, etc. There are many symptoms and cause of the TMD. One of the most common symptoms is the myofacial pain. This is the pain over the muscles that related to our jaw movements. The most common muscles are the muscles right underneath our ear lobes and all the way to the angle of the jaw. We call it masseter muscle, which has the common name of chewing muscle. Some patient might feel pain over the temporal area or at the back of the neck. Grinding is one of the habit that could cause the tension to the muscles. While unconsciously grinding during sleeping. Our muscles could be pretty tense though out the night. TMD can be caused by teeth grinding or clenching your teeth, and many other stress to the joint. Occlusal splints are a standard method to treat disc displacement with reduction of the temporomandibular joint (TMJ). We do provide Michigan Splint for TMD treatment at our office as well. Back to Invisalign with grinding or patient with TMD, we can design a patented device at the front teeth of your upper Invisalign aligners. Although, they are far different and not adjustable as the specialty designed Michigan splint, the bite ramps and the thickness of Invisalign trays could raise your bite a bit and dislocated the back teeth from biting tightly together. Thus, would reduce the tension of the muscles. I received positive feedback from many of my patients. My aligner-change is every 2-week. How does Invisalign work on my teeth (Day 0)? _Dr. Yang's Invisalign Journey (1) How does Invisalign feel while wearing them? In order to understand my patients' feeling, I order 5 sets of Invisalign aligners for myself. Here we go to start my short Invisalign adventure. ... I received my aligners today. It comes with a care pack. Inside the package, there is an instructional booklet, an Invisalign box, and some cleaning crystals. The Invisalign box is designed for you to store and secure your Invisalign trays. Please making sure that you are not going to wrap your trays inside the tissue paper or napkin. Many patients had lost their trays because of that, and a set of replacement aligners would cost you a good fortune. Te cleaning crystals could allow you soaking and disinfecting your aligners without discoloring them. While putting on my first set of aligners, the first thing I realized was its a bit hard to pronounce certain sounds, for instance, the "S", "T", "D", and even saying "Th"ank you could be quite challenge. From my patients' experience, most of them are able to speak fairly normal after three weeks. If you have to talk during work like me, I would suggest to perform some speech training ny reading a book word-by-word out loud. Many pt found this quite helpful. Other than pronunciation, my tongue also felt the taste of plastic while first insert the new trays, even the Invisalign trays are made with medical grade plastic material. I was surprised that the trays feel pretty strong and solid while putting into the mouth. Cause, they felt much more flexible while I delivered into patients' mouth. Other than these, the Invisalign aligners are pretty comfortable. How to prepare for applying to dental school while in undergrad?_ Dr. Yang's Dental School Application Series (3) As an undergraduate student, preparing for applying to dental school is like planning to run a marathon. It takes certain degree of the strategic planning process and lots of discipline. In this video, student doctor Elya Bach, our former pre-dental student, is sharing her opinions regarding to preparing from four different aspects: volunteering, research,.extrac...urricular activities, shadowing, and GPA. From her experiences, research is not a "requirement" from most of the schools, however, many students who apply do have research experiences. She would suggest all the potential applicants, pre-dental students, to identify a research project that you would really enjoy doing. Elya feels that the leadership position from the extracurricular activities is equally important. Many dental schools are looking for applicants with leadership characters that can work as a team. She also suggests the applicants to focus on few things that you are doing extremely well instead of list many different items in your application. As for shadowing, Elya mentioned that most of the schools require around 40 shadowing hours. However, while talking to other applicants, most of the students she talked to in average had more than 400 shadowing hours which stressed her out.
