In Watsonville, there are 4 clinics patients can go to for outpatient physical therapy. Just 4! One is hospital based, 1 is corporate owned, and the other 2 (one of them being me!) are privately owned small clinics, like mine. The Medicare cut will affect us all, and most importantly the patients we serve here in Watsonville. How? It decreases revenue for clinics, clinics that have a small profit margin to begin with, and likely don’t have a profit at all this year due to t...he Pandemic. So then the clinics have to figure out how to make up for that financial cut. Either they’ll let go of a therapist or 2, or they will ask therapists to increase their work load (see more patients in a day, for the same pay-which can decrease quality of care). They might stop seeing Medicare patients all together, or see them for less time. Overall, not good. Not only can this impact Medicare beneficiaries, but it will have a trickledown effect as most other major insurances (for example blue shield, Cigna, blue cross) end up implementing any cuts or changes Medicare does. In other words Medicare sets the bar. I want to ensure patients have access to care, and advocate for my profession, which I love, and you can help me! These girls . I haven’t been able to be at the kids classes much because of physical therapy related work and schoolwork, but I sure do miss them (ALL the kids!) I’ll be back as soon as I can. BuT.... I have some news I have a few things in the works to help encourage and empower our girls and women students, and also to help the school bring in additional financial resources to offset some of the set backs we have had to endure over the past few months. What is this for you wonder ? While I try not to talk anatomy to my patients too much (unless requested) so that there’s not so much emphasis on pathoanatomy, sometimes visuals do help, like when you mention the pelvic floor to someone who has never heard of it. It’s in there, in all of us, it’s part of our core and works with our breath. While I don’t specialize in pelvic physical therapy, I do screen for pelvic floor dysfunction, so that I can refer patients to those physi...cal therapists that do. This is one of the papers I have read the most. I think in the past 10 years I have read it at least once a year, and 3x in the past year. If you are skipping over the methods section of a research paper, or just reading the conclusion and applying it in practice, like I was after I graduated in 2009 with my Master’s, well I suggest you read this article, and the one that came before it. I didn’t learn much about critically appraising evidence back in grad school 10+ years a...go, despite doing a thesis in which I wrote my own methods section. It just wasn’t in the curriculum with enough depth for it to make an impact that would carry into my future practice. After all, critically appraising evidence takes work, you have to practice it, and either I didn’t know what to do or I *thought* I didn’t have the time. I was introduced to this piece of work while working on my doctorate-8 years ago geez time flies! Things like the power of a study, confidence intervals, number needed to treat, absolute and relative risk, effect size, ANOVA, correlation coefficientsthese are words clinicians should be familiar with, perhaps not experts, but at least familiar. Were the results valid? Are the patients in this study similar to the patients I see in my clinic? These are questions we need to ask of studies. A study can have statistically significant results, yet the results may not be clinically meaningful (confusing, I know!). Knowing when a study can be implemented into your practice is important, and it should consist of more than looking for studies that support your view (in other words it should not biased). Also, you don’t have to critically appraise every piece of evidence either, but to have the skill to do so when needed or desired is what I’m talking about. Most people don’t know that we (PTs) do this kind of work since most people think work for a PT means stretching a body part or telling you to #squatbenchdeadlift in my case lol, but so much more goes on behind the scenes. THIS is why I am in a fellowship, for the behind the scenes stuff, for the self-reflection and for the continued growth so that I can deliver more efficient and effective care. #physicaltherapy

CONTINUOUS GLUCOSE MONITOR - because #preventionfirst Mejor prevenir que curar . My dad is pre-diabetic. It's not easy to change someone’s ways, specially when there are no symptoms (yet), and when that someone has a sweet tooth (oh lord does he ever). So I got my dad this CGM to help me. It shows him what his blood glucose levels are, and what the effects of exercise and what he eats has on his glucose levels. . Now that he can SEE what’s happening on the app, he now un...derstands why 50g of carbs from rice alone is not the best thing for him (you know Colombians and their "arroz con papa with every meal lol), which I tried to explain but now he can actually see the spike it causes. I’ve asked him to monitor it often, before and after meals, and to take note of what he eats too so he can see which foods result in spike and which do not. So far he is eating less sweets and exercising more! . I wish his MD would’ve rx it (it’s expensive, but a lot less so with a rx), but nooooo, because he’s not a full blown diabetic yet. Why do we not emphasize prevention in this medical system? Doesn’t make sense. 1 in 3 Americans are prediabetic, and most of them (like 80%) don’t even know it.
