CDC Updates Flu Vaccine recommendations for 2019 - 2020 season.... (CDC)CDC has released its recommendations for influenza vaccination for the 2019-2020 season. Routine annual influenza vaccination continues to be recommended for all people 6 mo of age who don’t have contraindications. No preference is given to any one vaccine product when multiple products may be appropriate. Among the changes this year:... -> Afluria Quadrivalent, previously licensed for individuals 5 yo, is now recommended in children 6 mo. The dose volume is 0.25 mL per dose in children ages 6-35 mo, and 0.5 mL per dose for those 36 mo. -> Fluzone Quadrivalent can now be dosed at either 0.25 mL or 0.5 mL per dose in children 6-35 mo. This is not a new hypothesis about these drugs, which are used to treat a wide range of conditions from depression to epilepsy to incontinence. But the study, published in the journal JAMA Internal Medicine, is large, and it analyzed the use of the medications in more detail and over a longer period of time than many previous studies. Here’s what is known about the potential link between anticholinergic medicines and dementia: Which drugs are we talking about? Anticholinergic drugs include the antipsychotic clozapine; the bladder drug darifenacin (marketed as Enablex); the anti-nausea drug scopolamine; the bronchodilator ipratropium; the muscle relaxant tizanidine; antihistamines such as diphenhydramine (brand names include Benadryl), and antidepressants such as paroxetine (brand names include Paxil). These medications work by blocking a chemical called acetylcholine, which acts as a neurotransmitter and is involved in many nervous system functions including muscle movements, heart rate, the widening of blood vessels, respiratory functions and muscle contractions in the stomach during digestion. FDA adds Boxed Warning for increased risk of death with gout medicine Uloric (febuxostat) The U.S. Food and Drug Administration (FDA) has concluded there is an increased risk of death with Uloric (febuxostat) compared to another gout medicine, allopurinol. This conclusion is based on our in-depth review of results from a safety clinical trial that found an increased risk of heart-related death and death from all causes with Uloric. Uloric was FDA-approved in 2009 to treat a t...ype of arthritis called gout in adults. Gout happens when a naturally occurring substance in the body called uric acid builds up and causes sudden attacks of redness, swelling, and pain in one or more joints. Uloric works by lowering uric acid levels in the blood. When Uloric was approved in 2009, it was included in Warning and Precaution regarding possible cardiovascular events in patients treated with Uloric in the current prescribing information and required the drug manufacturer, Takeda Pharmaceuticals, to conduct a large postmarket safety clinical trial. The trial was conducted in more than 6,000 patients with gout treated with either Uloric or allopurinol. The results showed that overall, Uloric did not increase the risk of these combined events compared to allopurinol. However, when the outcomes were evaluated separately, Uloric showed an increased risk of heart-related deaths and death from all causes. In patients treated with Uloric, 15 deaths from heart-related causes were observed for every 1,000 patients treated for a year compared to 11 deaths from heart-related causes per 1,000 patients treated with allopurinol for a year. In addition, there were 26 deaths from any cause per 1,000 patients treated for a year with Uloric compared to 22 deaths per 1,000 patients treated for a year with allopurinol. Questions Answered: Latent TB infection & TB Diseae: Not everyone infected with TB bacteria becomes sick. As a result, two TB-related conditions exist: latent TB infection and TB disease.... Latent TB Infection: TB bacteria can live in the body without making you sick. This is called latent TB infection. In most people who breathe in TB bacteria and become infected, the body is able to fight the bacteria to stop them from growing. People with latent TB infection: Have no symptoms Don’t feel sick Can’t spread TB bacteria to others Usually have a positive TB skin test reaction or positive TB blood test May develop TB disease if they do not receive treatment for latent TB infection Many people who have latent TB infection never develop TB disease. In these people, the TB bacteria remain inactive for a lifetime without causing disease. But in other people, especially people who have a weak immune system, the bacteria become active, multiply, and cause TB disease. TB Disease: TB bacteria become active if the immune system can’t stop them from growing. When TB bacteria are active (multiplying in your body), this is called TB disease. People with TB disease are sick. They may also be able to spread the bacteria to people they spend time with every day. Many people who have latent TB infection never develop TB disease. Some people develop TB disease soon after becoming infected (within weeks) before their immune system can fight the TB bacteria. Other people may get sick years later when their immune system becomes weak for another reason. For people whose immune systems are weak, especially those with HIV infection, the risk of developing TB disease is much higher than for people with normal immune systems.
