There has been a lot of discussion about possible side effects to the coronavirus vaccine. The FDA recently advised that people with cosmetic fillers may develop swelling in areas that were previously injected. The vaccine mounts an immune response to build immunity to coronavirus. This likely causes an immune reaction to previously placed filler which can potentially cause temporary swelling. The swelling can be treated with steroids or antibiotics. The bottom line is that this side effect is minor and not a reason to avoid getting the vaccine. Just something to be aware of. Speak to your board-certified dermatologist if you have had filler and develop a reaction or have any concerns. Been playing around with the slo-mo feature! I know this is totally a laser geek video, but I was so excited when I could see both sprays of cooling. Look closely and you can see the cooling spray right before the laser pulse, then the red light flash of the Alexandrite laser, then the second spray of cooling immediately after the pulse. Here I was using the Gentlemax Pro for laser hair removal. This device uses a Dynamic Cooling Device which was invented by my friend & col...league, Dr. Stuart Nelson. The DCD uses tetrafluoroethane to cool and protect the epidermis while confining the heat from the laser within the hair follicle (where we want it). It also makes the treatment more comfortable. There are other ways to cool the skin. Contact cooling, usually with a sapphire crystal, or bulk cooling, i.e. a spray chiller (like the Zimmer) are other methods. Cooling can be applied before the laser pulse, during the laser pulse, or after the laser pulse. This video is an example of cooling before & after the laser pulse. Regardless of the method, the purpose of cooling is to keep the skin safe from burning and confining the heat from the laser to the target structure only. And as I mentioned, it makes the treatment more comfortable. Laser tip: with devices that use DCD, always hold the handpiece perpendicular to the skin so that the cooling fires in the same direction as the laser. Quick laser lesson. Nonablative lasers heat the skin while ablative lasers vaporize tissue. Nonablative lasers generally require a series of treatments while ablative lasers are more dramatic and may only require one treatment. The trade is the downtime. Artificial intelligence is becoming increasingly more common in every field. This video shows the first robotic arm that is capable of delivering a filler treatment plan designed by the provider. Melasma is a chronic condition worsened by sun exposure and female hormones. A combination approach to treatment is most effective. 1. The # 1 most important thing you can do is wear sunscreen every day. No matter what! Even if you think you don’t go out in the sun. Think windows. 2. Bleaching creams are also effective at lightening the brown patches. I prescribe compounded 12% hydroquinone with 6% kojic acid cream. 3. Other treatments that can help are chemical peels an...d low energy lasers. Most lasers make melasma worse, so make sure whoever is treating you is knowledgeable of your condition. 4. Due to the hormonal component, coming off your birth control will help, if possible. 5. Heliocare is an antioxidant from a fern extract available over the counter that acts as an internal sunscreen. It doesn’t replace your topical sunscreen, but adds extra protection. 6. The game changer for melasma is tranexamic acid. This is a prescription normally prescribed for heavy menstrual bleeding. It treats melasma by decreasing pigment production. There are risks which require a consultation to discuss whether you are a good candidate.
