Join The Basics of BED workshop at the Binge Eating Disorder Association’s Annual ConferenceBEDA 2016: Many Paths, One Journeyin San Francisco, Thursday, October 27th 1:00 - 3:00 pm PST. The workshop will educate about all aspects of BED to provide a positive framework and understanding. Lauren Ozbolt, MD, CEDS, Medical Director at Clementine (a Monte Nido affiliate for adolescents), writes about evaluating psychiatric medications in adolescents: " AFTER YEARS OF TREATING adolescents with eating disorders, I know the word "psychiatric medications" often sends concerns to teens and parents alike. It is true some adolescents have been scarred by memories of taking medications and simultaneously find many teens are terrified of psychotropic medications and h...ave false preconceived notions about how they work. As one young girl timidly told me, "I'm concerned I will be a zombie." The truth is, psychotropic medications can be very helpful in the treatment of certain eating disorders targeting such symptoms as anxiety, depression, obsessive thoughts and food preoccupations. But there is a point in time at which I find it most helpful to introduce. Thus, upon meeting a new adolescent at Clementine, I typically don't broach the idea of medications for the first few weeks of treatment so long as they are psychiatrically stable. I find it is much more valuable at this stage of the game to establish rapport. I want to take the time to get to know the patient, focus on the nutritional aspects of treatment and try to gain a better understanding of the function of the patient's eating disorder. After seeing the adolescent daily for several sessions and establishing rapport, I may then weave in the idea of medication, if warranted. I spend a great deal of time providing education about the medication, how it works in the body, risks, benefits and side effects. I will often tell the adolescent what they will view if they Google the medication - the positive and the concerns - and why or why not these presentations may apply to them. I give the teen some space and time to consider this approach and encourage them to discuss it with their parents. I similarly give parents a similarly extensive psychoeducation and detailed rationale for my recommendation. I don't believe in trying to convince an adolescent who is in the separation-individuation stage of development where opposition is the rule, to take a medication. Instead, I support the adolescent in their decision all the while, leaving the option open. I find this open approach - coupled with a relationship that fosters trust and mutual respect - sets a firm foundation for change and movement in the treatment. Psychiatry is an integral part of the treatment at Clementine. Twenty four hour access to psychiatrists is a hallmark of Clementine, and ensures the safety of each adolescent.
